Assessment of Treatment Strategies to Achieve Hepatitis C Elimination in Canada Using a Validated Model

Abstract: IMPORTANCEAchievement of the World Health Organization (WHO) target of eliminating hepatitis C virus (HCV) by 2030 will require an increase in key services, including harm reduction, HCV screening, and HCV treatment initiatives in member countries. These data are not available for Canada but are important for informing a national HCV elimination strategy. OBJECTIVE To use a decision analytical model to explore the association of different treatment strategies with HCV epidemiology and HCV-associated mortality … Show more

“…Canada and many developed countries have recently removed fibrosis-based restrictions on treatment coverage, however, treatment uptake needs to be scaled up quickly to realize the full benefits of DAAs in reducing mortality. 38 The assessment of risk factors such as cirrhosis, problematic alcohol consumption and diabetes was based on diagnostic codes, as in other similar studies, which could lead to misclassification of these conditions. 15,16 To make these definitions more specific and reduce misclassification, we required at least 1 hospitalization or 2 medical visits and/or prescription drugs specific to the condition such as anti-diabetic medications to define these conditions.…”

“…We utilized a mathematical model developed by the Centre for Disease Analysis [4] to model the current progress towards HCV elimination as well as the effect of various interventions on HCV-associated outcomes. This model has been used extensively in various healthcare situations and countries [9][10][11][12][13][14]. Briefly, the Excel-based Markov model forecasts the future HCV-infected population and associated liver-related morbidity (decompensated cirrhosis and hepatocellular carcinoma) and mortality.…”

“…Governments worldwide have committed to achieving the World Health Organization (WHO) targets to eliminate the hepatitis C virus (HCV) as a public health threat by 2030 (1). Although recent modelling studies suggest that some countries, including Canada, are on track to eliminate HCV, they could not account for the emergence of the coronavirus disease 2019 (COVID- 19) pandemic, which may jeopardize progress toward elimination (2,3).…”
