Association of Aspirin With Prevention of Venous Thromboembolism in Patients After Total Knee Arthroplasty Compared With Other Anticoagulants

Abstract: IMPORTANCE There has been significant debate in the surgical and medical communities regarding the appropriateness of using aspirin alone for venous thromboembolism (VTE) prophylaxis following total knee arthroplasty (TKA). OBJECTIVE To determine the acceptability of aspirin alone vs anticoagulant prophylaxis for reducing the risk of postoperative VTE in patients undergoing TKA. DESIGN, SETTING, AND PARTICIPANTS Noninferiority study of a retrospective cohort of TKA cases submitted to the Michigan Arthroplasty … Show more

“…28,29 As an example, in a very recent report, Hood et al described a noninferiority study of a retrospective cohort of TKR cases in hospitals ranging from small community hospitals to large academic and nonacademic medical centers. 30 The study included 41,537 patients who underwent primary TKR. Methods of pharmacologic prophylaxis were different: neither aspirin nor anticoagulants for 668 patients (1.6%), aspirin only for 12,831 patients (30.9%), anticoagulant only (e.g., LMWH, warfarin, factor Xa inhibitors) for 22,620 patients (54.5%), and both aspirin and anticoagulant for 5,418 patients (13.0%).…”

“…While there have been numerous studies examining the risk of VTED in primary total knee arthroplasty (TKA), far fewer studies have investigated how that risk compares to other knee arthroplasty procedures, such as unicompartmental knee arthroplasty (UKA) and revision TKA [ 1 , 2 , 12 17 ]. Given the paucity of reliable comparative studies comparing the VTED risk of UKA and revision TKA to primary TKA, we designed a large-scale, multivariate analysis using a national database with recent data.…”
