Association of Long-term, Low-Intensity Smoking With All-Cause and Cause-Specific Mortality in the National Institutes of Health–AARP Diet and Health Study

Abstract: IMPORTANCE A growing proportion of US smokers now smoke fewer than 10 cigarettes per day (CPD), and that proportion will likely rise in the future. The health effects of smoking only a few CPD over one's lifetime are less understood than are the effects of heavier smoking, although many smokers believe that their level is modest.OBJECTIVE To evaluate the associations of long-term smoking of fewer than 1 or 1 to 10 CPD (low intensity) with all-cause and cause-specific mortality compared with never smoking cigar… Show more

“…Notably, among individual CVH metrics, smoking, diet, physical activity, FPG levels, and BP contributed to the reduction in risk of both all‐cause and CV mortality. Previous quantitative reviews suggested that smoking, an established risk factor, has been proven to exert adverse effects on mortality . In addition, physical activity and diet quality have been associated with reduced risk of CVD and mortality .…”

“…Previous quantitative reviews suggested that smoking, an established risk factor, has been proven to exert adverse effects on mortality. 24,25 In addition, physical activity and diet quality have been associated with reduced risk of CVD and mortality. 26,27 Levels of HbA 1c have been recognized as predictors of allcause mortality.…”

“…However, a previous longitudinal study showed that people reported past CPD with good validity for 20 years earlier (kappa = 0.63) and fair validity for 32 years earlier (kappa = 0.36) . In the NIH‐AARP cohort, we also observed good concordance of recalled smoking after 10 years in the present study (74% concordance among lifelong consistent smokers of 10 or fewer CPD) . Despite our large sample size, we were underpowered to estimate associations between lifelong low‐intensity smoking and individual cancers.…”

“…We determined age at smoking initiation and duration of smoking based on this information. We previously reported that 74% of participants who reported lifelong smoking of 10 or fewer CPD in the 2004–2005 questionnaire also reported currently smoking 10 or fewer CPD, and 24% reported smoking 11 to 20 CPD at the time of the 1995–1996 questionnaire …”
