Association of retinol binding protein4 (RBP4) and ghrelin plasma levels with insulin resistance and disease severity in non‐diabetic patients with hidradenitis suppurativa

Abstract: Hidradenitis suppurativa (HS) is a chronic, inflammatory and skin disease of the hair follicle. The pathogenesis of HS remains largely uncertain, although current evidence suggests the existence of an early phase of immune activation with progression to chronic inflammation. [1] In this sense, several pro-inflammatory cytokines, such as tumor necrosis factor-alpha (TNF-α), interleukin (IL)-1β, IL-10 and IL-17, are considered to play a pivotal role in the HS pathogenic process. [1,2] Besides, some adipokines th… Show more

“…In the current study and other studies, there was no significant correlation between ghrelin levels and anthropometric or metabolic variables. These studies included non-diabetic adolescents and healthy adults ( 28 , 43 ). In addition, earlier studies reported that exogenous ghrelin administration induced insulin resistance in rats and humans ( 44 , 45 , 46 ).…”

“…RBP4 is also a transport protein for free vitamin A [ 56 ]. In a cross-sectional study of 137 patients (77 HS patients and 60 controls) without diabetes mellitus, higher RBP4 and lower ghrelin levels were found in HS [ 57 ]. RBP4 levels correlated positively with disease severity and insulin resistance in HS patients independent of BMI [ 57 ].…”

“…[49,50] This concept is alos in line with newly recognized endocrine abnormalities associated with hidradentitis suppurative/ acne inversa. Gonzaléz-López et al [51] who found that low plasma levels of ghrelin, a predominant stomach-derived hormone, as well as high plasma levels of retinol-binding protein-4, may serve as novel biomarkers for insulin resistance in hidradenitis suppurativa indicating an increased risk. This is further elaborated exemplarily by Briganti et al [52] who examine acne as the phenotypic result of altered dermato-endocrine responses of human skin, and by Zouboulis, [53] who argues in a provocative and highly stimulating essay that the coordinated activities of endocrinological, metabolic and immunological/inflammatory factors drive acne pathogenesis, with peroxisome proliferator-activated receptors functioning as a putative regulatory hub, in line with similar arguments that have been developed for the role of these receptors in human hair follicle physiology and pathology.…”
