COVID-19, the disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has threatened public health worldwide. Host antiviral immune responses are essential for viral clearance and disease control, however, remarkably decreased immune cell numbers and exhaustion of host cellular immune responses are commonly observed in patients with COVID-19. This is of concern as it is closely associated with disease severity and poor outcomes. Human leukocyte antigen-G (HLA-G) is a ligand for multiple immune inhibitory receptors, whose expression can be upregulated by viral infections. HLA-G/receptor signalling, such as engagement with immunoglobulin-like transcript 2 (ILT-2) or ILT-4, not only inhibit T and natural killer (NK) cell immune responses, dendritic cell (DC) maturation, and B cell antibody production. It also induces regulatory cells such as myeloid-derived suppressive cells (MDSCs), or M2 type macrophages. Moreover, HLA-G interaction with CD8 and killer inhibitory receptor (KIR) 2DL4 can provoke T cell apoptosis and NK cell senescence. In this context, HLA-G can induce profound immune suppression, which favours the escape of SARS-CoV-2 from immune attack. Although detailed knowledge on the clinical relevance of HLA-G in SARS-CoV-2 infection is limited, we herein review the immunopathological aspects of HLA-G/receptor signalling in SARS-CoV-2 infection, which could provide a better understanding of COVID-19 disease progression and identify potential immunointerventions to counteract SARS-CoV-2 infection.

This study aims to discover how HLA-G gene variations and sHLA-G, as well as other host genetic factors, can affect SARS-CoV-2 infection. In this case-control study, 67 Patients with COVID-19 were and 65 healthy controls were genotyped for their main functional polymorphisms in the exon 8 of the 3' untranslated regions (3' UTRs) using sanger sequencing. Associations were assessed for five inheritance models (co-dominant, dominant, recessive, over-dominant and log-additive). Moreover, the levels of plasma soluble HLA-G (sHLA-G) were explored using ELISA method. Our results revealed all-main polymorphism in the 3’UTR region previously described for the HLA-G 3’UTR, namely, the 14-bp Ins/Del (rs371194629), +3003C/T (rs1707), + 3010C/G (rs1710), + 3027A/C (rs17179101), + 3035C/T (rs17179108), + 3142C/G (rs1063320), + 3187A/G (rs9380142) and + 3196C/G (rs1610696). The 14-bp INS/DEL polymorphism was strongly associated with COVID-19 symptoms development for almost all tested inheritance models (P < 0.001). Inversely, the (+3196C/G) polymorphism exhibited a protective effect against COVID-19. Three haplotypes; UTR-1, UTR-3, and UTR-5 were found associated with COVID-19 symptoms (P < 0.05), The level of HLA-G in the serum was significantly higher in COVID-19 individuals than in healthy individuals (P <0.001).These findings suggest that HLA-G gene polymorphisms in the regulatory 3’UTR region of the HLA-G gene may influence the host immune response to SARS-CoV-2 infection. A deeper comprehension of host genetics could be useful in identifying high-risk individuals and in the development of better treatments or vaccinations.

Hepatitis C virus (HCV) infection is a major cause of chronic liver disease. Chronic HCV infection is also an important cause of hepatic fibrosis, cirrhosis and hepatocellular carcinoma (HCC). HCV has the capacity to evade immune surveillance by altering the host immune response. Moreover, variations in immune‐related genes can lead to differential susceptibility to HCV infection as well as interfere on the susceptibility to the development of hepatic fibrosis, cirrhosis and HCC. The human leucocyte antigen G (HLA‐G) gene codes for an immunomodulatory protein known to be expressed in the maternal–foetal interface and in immune‐privileged tissues. The HLA‐G 3′ untranslated region (3′UTR) is important for mRNA stability, and variants in this region are known to impact gene expression. Studies, mainly focusing in a 14 bp insertion/deletion polymorphism, have correlated HLA‐G 3′UTR with susceptibility to viral infections, but other polymorphic variants in the HLA‐G 3′UTR might also affect HCV infection as they are inherited as haplotypes. The present study evaluated HLA‐G 3′UTR polymorphisms and performed linkage disequilibrium test and haplotype assembly in 286 HCV infected patients who have developed fibrosis, cirrhosis or HCC, as well as in 129 healthy control subjects. Haplotypes UTR‐1, UTR‐2 and UTR‐3 were the most observed in HCV+ patients, in the frequencies of 0.276, 0.255 and 0.121, respectively. No statistically significant difference was observed between HCV+ and control subjects, even when patients were grouped according to outcome (HCC, cirrhosis or fibrosis). Despite that, some trends in the results were observed, and therefore, we cannot rule out the possibility that variants associated to high HLA‐G expression can be involved in HCV infection susceptibility.
