Association of Vision Loss in Glaucoma and Age-Related Macular Degeneration with IADL Disability

Abstract: VA loss in AMD and severe VF loss in glaucoma are associated with self-reported difficulties with IADLs. These limitations become more likely with increasing magnitude of VA or VF loss.

“…9 Several populationbased and clinical studies have evaluated self-reported disability and impaired VRQOL that result from having glaucoma. [10][11][12][13][14][15][16][17] Patients with bilateral glaucoma and greater VF loss have poorer patient-reported VRQOL (pVRQOL) and more difficulties in daily activities including driving, dependence on others, social functioning, mental health, general vision, distance vision, near vision, color vision, and peripheral vision. 12,14,15 Only a few reports discuss the impact of the VF defect location on various pVRQOL domains.…”

“…Detailed descriptions of inclusion criteria for each group are described elsewhere. 10,20,21 Briefly, glaucoma suspect control subjects were required to have a medical record diagnosis of glaucoma suspect and minimal VF loss (better-eye mean deviation [MD] better than −3 dB). Subjects with glaucoma were required to have a medical record diagnosis of glaucoma and abnormal right and left eye VFs with MDs worse than −3 dB.…”

“…23 Specifically, the presence of vision loss in AMD and field loss in glaucoma were associated with functional disability. 24 The presence of AMD was an independent risk factor for functional disability and impaired ADL performance, after adjusting for walking disability, hospital admissions, and other comorbidities. 25 In a year-long analysis of over 1000 individuals aged 65 or older living in the community, those with visual impairment were 2.5 times more likely to experience functional decline than unimpaired elderly patients after adjusting for age, sex and cognitive status (95% CI 1.37–4.48).…”
