Atypical penile Mondor’s disease – involvement of the circumflex vein

Abstract: Mondor's disease is a rare condition characterised by superficial thrombophlebitis of subcutaneous veins, most commonly over the anterior chest wall. Penile Mondor's disease is rarer, arising out of thrombophlebitis of the penile veins. It typically involves the dorsal vein of the penis and presents with a cord-like indurated lesion with a beaded feel, palpable along the length of the involved vein. Though penile Mondor's disease involving the dorsal vein of the penis has been reported by many authors, we repo… Show more

“…In PMD, the induration appears on the dorsal and dorsolateral aspects of the penis due to the involvement of an inferior part of the superficial vein system of the abdominal wall ( 16 ) ( Figure ). The circumflex vein can also be affected, which some authors claim to be a case of atypical PMD ( 28 ).…”

“…In the present study, 25 patients with hardened lesions, in arciform disposition, located around the balanopreputial groove, similar to Mondor’s disease (MD) in an atypical location, are reported. 8 , 9 Among these 25 patients, 19 had a recent clinical and laboratory diagnosis of syphilis. Six of these 19 cases presented an evolution that ranged from 3 to 30 days, with negative serology at the first consultation, and positive after one week.…”

“…3 Doppler is useful, especially in atypical cases that involve other superficial veins, such as the circumflex vein of the penis. 4 According to the histology, MD can present with 4 phases: thrombus formation, thrombus organization, recanalization and thick-walled fibrosis. It is mainly associated to traumas, sexual intercourse or vigorous physical exercises, use of vacuum devices or rings for the compression of the base of the penis.…”
