Atypical Presentation of Peripheral Ossifying Fibroma in the Mandible

Abstract: Peripheral ossifying fibroma (POF) is a localized reactive enlargement of the gingiva often associated with the papilla and originate from underneath the periodontium. POF occurs predominantly in females, especially in the anterior maxillary region of young adults. The histopathological examination provides a confirmatory diagnosis of such lesions due to their heterogeneous clinical and radiographic characteristics. This case report describes an atypical presentation of POF in the anterior mandible in an adult… Show more

“…The differential diagnoses in many studies included fibrous hyperplasia, pyogenic granuloma and peripheral giant cell granuloma and peripheral odontogenic fibroma [ 4 ] and osteosarcoma [ 14 ]. In our case, because of the radiographic findings especially the presence of radiopacity, we excluded most of those differential diagnoses and suspected a peripheral ossifying fibroma.…”

“…It is a non-neoplastic reactive lesion, which can be soft with a smooth surface. Statistics state that 60% of the lesions occur in the maxillary anterior region [ 1 ]. The colour of the lesion can vary from light rosy colour to dark cherry red colour.…”

“…Additional terminology utilized to describe the POF include a peripheral cementifying fibroma, a peripheral fibroma with cementogenesis, a peripheral fibroma with osteogenesis, a peripheral fibroma with calcification, calcified or ossified fibrous epulis, and calcified fibroblastic granuloma [3,4]. The diagnosis of the POF is challenging due to its intricate clinical and histological features, which exhibit similarities with various different disorders, including fibroma, inflammatory fibrous hyperplasia, pyogenic granuloma, peripheral giant cell granuloma, and non-bone metastases of some cancers [5,6]. The aim of this study is to present a rare case of a maxillary POF, where the last two teeth of the mandible irritated the edentulous maxilla, triggering the formation of the POF.…”
