Atypically large well-differentiated hepatocellular carcinoma with extensive fatty metamorphosis: Report of a Case

Abstract: : A large well-differentiated hepatocellular carcinoma (HCC) with fatty change is rare, and to date only a few cases have been reported. Herein, we present a 68-year-old man who developed a well-differentiated HCC with extensive fatty metamorphosis. The patient was referred to our institute because of a rapidly growing tumor in the left lobe of the liver. Ultrasonography showed a hyperechoic lesion with a peripheral hypoechoic area. Dynamic contrast-enhanced computed tomography (CT) scan in all three phases re… Show more

“…6,7 In some reports, this fatty metamorphosis was more frequent in small, well-differentiated hepatocellular carcinomas; however, with increased tumor diameter and increased histological grade, the fatty changes became infrequent. 6,7 The fatty component of these tumors also can be detected by imaging techniques, such as ultrasonography or computed tomography. 8 In some reports, fatty changes in hepatocellular carcinoma are more frequent in Asian than in non-Asian populations.…”

“…[6][7][8] Although the cause of hepatocellular carcinoma with fatty change has not been well understood, it is also considered to be related to ischemia and metabolic disorders. 6,7 In some reports, this fatty metamorphosis was more frequent in small, well-differentiated hepatocellular carcinomas; however, with increased tumor diameter and increased histological grade, the fatty changes became infrequent. 6,7 The fatty component of these tumors also can be detected by imaging techniques, such as ultrasonography or computed tomography.…”

“…This fatty transformation is more common in tiny, well-differentiated HCC nodules, particularly those with a diameter of less than 20 mm. A well-differentiated HCC with fatty alteration is quite uncommon to progress to over 30 mm [ 6 ].…”

“…4,5 Few case reports and no case series focusing on atypical large well-differentiated HCC (atypical-HCC) have been published. 6,7 Therefore, clinical characteristics and outcome of patients with atypical-HCC are ill-defined.…”
