Autoimmune reactivity in Cogan's syndrome: A preliminary report

Abstract: Autoimmune inner ear disease was first described by McCabe in 1979. The diagnosis is supported by cell-mediated immune responses to inner ear membrane antigen stimulation. Cogan's syndrome consists of vestibuloauditory dysfunction, ocular inflammation, and nonreactive serologic tests for syphilis. The ocular disease can be controlled by steroids; unfortunately, some patients ultimately become totally deaf. An autoimmune etiology has long been suspected but only recently confirmed by McCabe. In our study two pa… Show more

“…The most commonly accepted hypothesis-an autoimmune mechanism 6,7,17,18 -is supported by the frequently successful remission of hearing loss after steroid administration, the positive transformation of the lymphoblastic test regarding cochlear antigen in some cases, 19,20 and the association of Cogan's syndrome with other autoimmune diseases such as rheumatoid arthritis. [21][22][23] Schuknecht and Nadol 24 published a histologic study of the temporal bone from a patient who had suffered from Cogan's syndrome and later died of another pathology.…”

“…Ototoxic antibiotics, meningitis, tumours, bilateral endolymphatic hydrops and systemic autoimmune disorders (e.g. lupus erythematosus, polychondritis, Cogan syndrome [12], rheumatoid arthritis) are among the many causes of BV. Despite comprehensive investigations, its aetiology still remains obscure in about 20-30% of cases (so-called idiopathic bilateral vestibulopathy, IBV).…”
