Introduction: Hypothyroidism has long been known for its effects on different organ systems, leading to hypometabolism. However, subclinical hypothyroidism, its most prevalent form, has been recently related to cardiovascular risk and also to maternal-fetal complications in pregnant women. Objectives: In these clinical practice guidelines, several aspects of this field have been discussed with the clear objectives of helping physicians treat patients with hypothyroidism, and of sharing some of our Latin American-based clinical experience. Materials and methods: The Latin American Thyroid Society commissioned a Task Force on Hypothyroidism to develop evidence-based clinical guidelines on hypothyroidism. A systematic review of the available literature, focused on the primary databases of MedLine/PubMed and Lilacs/SciELO was performed. Filters to assess methodological quality were applied to select the best quality studies. The strength of recommendation on a scale from A-D was based on the Oxford Centre for Evidence--based Medicine, Levels of Evidence 2009, allowing an unbiased opinion devoid of subjective viewpoints. The areas of interest for the studies comprised diagnosis, screening, treatment and a special section for hypothyroidism in pregnancy. Results: Several questions based on diagnosis, screening, treatment of hypothyroidism in adult population and specifically in pregnant women were posed. Twenty six recommendations were created based on the answers to these questions. Despite the fact that evidence in some areas of hypothyroidism, such as therapy, is lacking, out of 279 references, 73% were Grade A and B, 8% Grade C and 19% Grade D. Conclusions: These evidence-based clinical guidelines on hypothyroidism will provide unified criteria for management of hypothyroidism throughout Latin America. Background: Vitiligo is an acquired depigmenting disorder due to destruction of melanocytes, many hypotheses have been suggested for its pathogenesis. One of these hypotheses suggest that there are autoimmune and endocrine dysfunction involvement. The involvement of vitiligo with thyroid autoimmune diseases, with the increased prevalence of auto antibodies including thyroid auto antibodies in vitiligo support the hypothesis. Patients and Methods: 50 Libyan patients under same age and gender with vitiligo, and 50 controls . Patients were excluded if they had a history of thyroid, or other autoimmune diseases. Data on age, onset of illness, duration and disease activity were determined . Serum T3, T4, TSH , and antibodies to TPO and TG were measured in both vitiligo patients & controls. All patients and control subjects underwent thyroid ultrasonography. Results: Fifty patients with vitiligo and their 50 matched controls were studied. More than half of the patients (52%) were females and 48% were males, their mean of age was 40 ± 11 years, and the duration of vitiligo was 11± 9 years. Vitiligo vulgaris type was the most common form seen in 68% of the patients, and 42% reported at least one family member affected with vitiligo. Family history of thyroid disorder was seen in 20% of the patients. Thyroid functional abnormalities were significantly seen more in patients than control subjects. The frequency of TG and TPO thyroid autoantibodies was significantly higher in vitiligo patients than in healthy controls (P < 0.01). Abnormal thyroid ultrasound study was seen in 18 (36%) of the patients compared to 6 (12%) of the control subjects (P < 0.05). Conclusion: Our findings pointed to a significant association between vitiligo and thyroid autoimmunity and showed that testing the level of thyroid autoantibodies is relevant in vitiligo patients.

Background: Vitiligo is associated with varying degrees of thyroid dysfunctions. This study was aimed to evaluate the thyroid dysfunctions in patients with vitiligo in Port Harcourt, Nigeria, West Africa.Methods: A retrospective, descriptive, cross-sectional analysis of thyroid function tests variables of 105 vitiligo patients who visited the Department of Chemical Pathology and Metabolic Medicine of the University of Port Harcourt Teaching Hospital between 1st January 2012 and 31st December 2016 was conducted. Records of age, sex, thyroid stimulating hormone, total thyroxine, total triiodothyronine was collected and analyzed using Shapiro-Wilk, descriptive, chi-square, Fisher’s exact, and Pearson’s tests. A statistical significance of p <0.05 was applied.Results: Females predominated among study cohort (61.9% female versus 38.1% male; p = 0.015). Most patients were less than 30 years old in both sexes. 26.7% (37 out of 105) had various degrees of thyroid dysfunctions. Among this 26.7% (37) with various thyroid dysfunctions, are 28 (75%) females and 9 (25%) males. These dysfunctions include primary hypothyroidism (6.7%), primary hyperthyroidism (1%), subclinical hypothyroidism (17.1%) and subclinical hyperthyroidism (1.9%), while the majority were euthyroid (73.3%). Subclinical hypothyroidism was the most common disorder (n = 18; 17.2%) with female preponderance (females n = 11; 61% versus males n = 7; 39%).Conclusions: This study confirms high frequency of thyroid dysfunctions among vitiligo patients. The culture of regular assessment of thyroid function should be mandatory among patients with vitiligo.
