Autologous hematopoietic stem cell transplantation suppresses Gd-enhanced MRI activity in MS

Abstract: These results demonstrate that the therapeutic sequence CY-BEAM-ASCT has the capacity to completely suppress MR-enhancing activity, an effect that is sustained with time. The final impact of this procedure on disease course remains to be established.

“…In our study, there were no new lesions captured by MRI in either conditioning regimen groups, showing the effectiveness in the suppression of MR-enhanced activity, as in the Italian study of the BEAM regimen. 57 Lesion enhancement by MRI was used in our study only as a marker for inflammatory activity for patient selection. In our sample, we did not find a correlation between improved MRI scans and the level of neurological disability (EDSS), as opposed to the Italian group.…”

“…A high EDSS score X6 (reflecting advanced disease in multiple sclerosis) or high pulmonary artery pressure (reflecting advanced disease in systemic sclerosis) were associated with poor outcome. 12,35,43 We did not attempt to define risk factors associated with specific causes of death, for example, hemorrhage, cardiac toxicity or veno-occlusive diseases. The numbers are too small, and the heterogeneity i.d.…”

“…In multiple sclerosis, flares of the disease have been observed during mobilization 45 during conditioning 12 and at the time of engraftment in the context of engraftment syndromes. 43 In systemic sclerosis, mortality appears to be correlated, especially with severe pulmonary artery hypertension. 10 However, successfully transplanted patients with moderate pulmonary hypertension may improve.…”

“…Spontaneous immune conditions in mice can be transferred by hemopoietic stem cells, which differentiate into T and B lymphocytes before the disease is manifest, leading to the hypothesis that the cause of immune diseases may lie in the hemopoietic stem cells, and that syngeneic bone marrow transplantation (in conjunction with an immunosuppressive treatment) induces a state of long-term unresponsiveness to myelin antigens presumably by a mechanism of tolerance that is antigen specific (9)(10)(11)(12)(13)(14). Among bone marrow cells, CD34 ϩ cells may be responsible for this effect directly or through their progeny, as exemplified by MS patients who received autologous CD34 ϩ cells (15).…”
