Automated treatment planning as a dose escalation strategy for stereotactic radiation therapy in pancreatic cancer

Abstract: Purpose: To assess the feasibility of automated stereotactic volumetric modulated arc therapy (SBRT-VMAT) planning using a simultaneous integrated boost (SIB) approach as a dose escalation strategy for SBRT in pancreatic cancer. Methods: Twelve patients with pancreatic cancer were retrospectively replanned. Dose prescription was 30 Gy to the planning target volume (PTV) and was escalated up to 50 Gy to the boost target volume (BTV) using a SIB technique in 5 fractions. All plans were generated by Pinnacle 3 Au… Show more

“…In this strategy, the planning optimization process uses a template to formulate all the planning goals and an iterative approach able to mimic the planning procedure usually adopted by experienced planners to generated high-quality plans. This approach has been investigated in several publications for prostate (21)(22)(23), headneck (24), and for extracranial stereotactic treatments (25,26) reporting an overall increase of plan quality together with a substantial reduction of planning time and interplanner variations.…”

“…This strategy is based on a template-based planning optimization procedure that employs an iterative approach capable of replicating all processes of experienced and skilled planners in order to generated high-quality plans ( 23 ). The Autoplanning module has been recently evaluated for SBRT treatment of liver and pancreatic tumors, showing a major ability to deliver ablative doses respecting all normal tissue constraints with respect to manually generated plans ( 24 , 25 ).…”

“…bladder and rectum) overlapped with the target volume. Utilisation of the AP software is widely reported across other anatomical sites including oesophagus 14 and breast 15 as well as applications for stereotactic body radiotherapy (SBRT) of the pancreas, 16 lung 17 and liver. 18 In this study, we evaluate the use of AP for prostate and prostate bed cases with or without pelvic lymph node irradiation.…”
