“…The role of contextual or environmental variables is a key policy issue, since researchers and policy‐makers are often interested in understanding the influence of health policies, or the organization of health services, on the use of formal services (Phillips et al 1998). The majority of studies that were found, with the exception of four (Snider 1980; Penning 1995; Shapiro & Tate 1997; Hawranik 1998), were conducted in an American context.
