ThresholdROC: Optimum Threshold Estimation Tools for Continuous Diagnostic Tests in R

Abstract: We introduce an R package that estimates decision thresholds in diagnostic settings with a continuous marker and two or three underlying states. The package implements parametric and non-parametric estimation methods based on minimizing an overall cost function, as well as confidence interval estimation approaches to account for the sampling variability of the cut-off. Further features of the package include sample size determination and estimation of diagnostic accuracy measures. We used randomly generated da… Show more

“…The aim in developing cutpointr was to make improved methods to estimate and validate optimal cutpoints available. While there are already several packages that aid the calculation of a wide range of metrics (López-Ratón et al 2014Perez-Jaume et al 2017), some of these rely on suboptimal procedures to estimate a cutpoint. Most use the empirical method, which is prone to high variability an to overestimating the diagnostic utility of optimal cutpoints.…”

“…Linear mixed models were used to assess the association between ASP activity and ammonium, albumin, and triglycerides to account for the correlation between the measurements from the same subjects. 26 We used the Youden index maximization method, 27,28 to estimate cutoffs for ammonium, triglycerides, and albumin that optimally discriminated between patients with low ASP activity (<100 IU/L) and therapeutic ASP activity (≥100 IU/L). The corresponding 95% confidence intervals were calculated using bootstrap.…”
