Bal kamrai deformáció cardialis könnyűlánc-amyloidosisban és hypereosinophilia-szindrómában. Eredmények a MAGYAR-Path Tanulmányból

Abstract: Bevezetés: A hypereosinophilia-szindróma (HES) és az immunglobulinkönnyűlánc-amyloidosis (ALA) két ritka hematológiai betegség, melyek cardialis eltérésekkel járnak együtt. Célkitűzés: A jelen vizsgálat célja a HES-és ALA-betegek bal kamrai (BK-i) deformációs paramétereinek összehasonlító vizsgálata volt háromdimenziós speckle-tracking echokardiográfia (3DSTE) segítségével. Módszer: A vizsgálatok során 10 HES-beteg (átlagos életkor: 60,9 ± 14,7 év) és 19 ALA-ban szenvedő páciens (átlagos életkor: 63,4 ± 7,8 év… Show more

“…Moreover, the effects of different risk factors and aging could also play a role with haemodynamic effects of the aorta and its stiffened walls [19]. Moreover, the above mentioned functional abnormalities of the LV and LA could also have effects on each other resulting in MA dilation and functional impairment, as well [7,9,10]. Due to left heart abnormalities, changes may be present in the right heart as well despite absence of any cardiovascular symptoms.…”

“…Aortic stiffness is increased in HES, but little is known about HES-related remodeling of heart chambers before the development of Loeffler endocarditis [ 7 , 8 , 9 , 10 , 19 ]. The novel echocardiographic technique, 3D echocardiography is a non-invasive method, and it seems to be optimal to quantify changes of atria and ventricles respecting the cardiac cycle [ 12 , 13 , 14 ].…”

“…The aim of our study was to evaluate the clinical value of 3DSTE in certain disorders like HES. The results from the MAGYAR-Path study presented several abnormalities in the valvular and myocardial mechanics in HES [ 16 , 17 , 18 , 19 , 20 , 21 ].…”

“…In a 2DSTE study, global LV-CS and global LV-RS were similar in normal controls and in patients with HES, but the global LV-LS of the normal controls was significantly higher than that of the patients with HES, and a global LV-LS ≤ 17% was the best predictor of LV endocardial dysfunction [ 31 ]. Results from the MAGYAR-Path study, in a comparison with matched healthy subjects, no LV dilation but a reduced 3DSTE-derived global LV-LS could be detected in patients with HES, localized to the basal regions [ 16 ]. Moreover, reduced LV apical rotation and twist were present in patients with HES, being mostly in the early necrotic phase.…”
