The present study is intended to retrospectively compare the short-and long-term outcomes of 3 different treatment methods in patients undergoing bariatric surgery and the variances in weight and nutritional parameters during the preoperative and postoperative periods. In this study, 534 patients who underwent laparoscopic sleeve gastrectomy (LSG), laparoscopic Roux-En-Y gastric bypass (LRYGB), and laparoscopic one anastomosis gastric bypass (LOAGB) between 2014 and 2021 were included. The sociodemographic and biodemographic characteristics of these patients, their weight losses and nutritional changes in the preoperative and postoperative periods, operative times, hospital stays, complications, and morbidity and mortality rates were retrospectively compared. There was a statistically significant difference between the surgical methods in the percentages of excess weight loss and total weight loss in the 1st and 3rd months. There were significant differences in the homeostasis model assessment of insulin resistance, folic acid, vitamin D, iron, ferritin, and parathyroid hormone levels (P < .05). All 3 techniques were found to be successful in facilitating weight loss at the end of the first year. LRYGB and LOAGB were found to be superior to LSG in terms of remission from diabetes during the first 6 months, whereas LSG was superior to the other methods in terms of nutritional deficiencies. Despite being more advantageous in terms of operative time, LSG and LOAGB were disadvantageous compared with the LRYGB technique because of the higher rates of leakage and mortality in the LSG technique and the higher rate of bile reflux in the LOAGB technique.

“…El número de procedimientos bariátricos ha ido en aumento debido a dos factores principalmente: el aumento en la prevalencia de sobrepeso y obesidad de la población y la evidencia que sugiere que estos procedimientos son el mejor tratamiento para lograr una pérdida de peso sostenida 1,2 . En la población mexicana el porcentaje de adultos mayores de 20 años con sobrepeso y obesidad es del 75.2% según lo descrito por Levi et al 3 ; Zerrweck et al refieren que los procedimientos bariátricos más frecuentemente realizados en México son el bypass gástrico en Y de Roux (BGYR) y la gastrectomía vertical en manga (MG) con 45.6 y 44.7% respectivamente 4 . A pesar de los amplios beneficios de la cirugía bariátrica, se estima que la mayoría de los pacientes sometidos a BGYR recuperarán entre el 20 y el 30% del peso que perdieron inicialmente y un tercio de los pacientes recuperaran casi todo el peso perdido, destacando como causas principales la dilatación de la gastro-yeyuno anastomosis y la fístula gastro-gástrica 5,6 .…”

El número de procedimientos bariátricos ha aumentado de manera importante en los últimos años. A la par de este incremento se encuentra el número de complicaciones asociadas a ellos, siendo una de las más importantes la reganancia de peso, la cual tradicionalmente se ha tratado con cambios en el estilo de vida y que cuando falla requiere de la cirugía de revisión que presenta alta morbilidad. Recientemente se han descrito procedimientos endoscópicos para la reganancia de peso posterior a un procedimiento bariátrico que intentan disminuir la morbilidad asociada a la cirugía de revisión conservando la tasa de éxito. La coagulación con plasma de argón es una de las que mejores resultados ha presentado y en esta revisión exploramos sus indicaciones, técnica y las comparaciones con otras técnicas endoscópicas.Palabras clave: Cirugía bariátrica.
