Behcet Extracranial Carotid Aneurysms: Is There Still a Role for Ligation?

Abstract: Surgical repair in Behcet ECAAs should be performed with special precautions in order to prevent anastomotic pseudoaneurysm formation. Ligation can be performed when the anatomical and/or pathological circumstances are not favourable, provided that the stump pressure is adequate to maintain cerebral perfusion.

“…In that case, medical treatment with pulsed cyclophosphamide should be the initial step, as dramatic improvement of aneurysms has been reported with this treatment [61,62]. Successful surgical interventions also have been reported [63]. However, in the case of classic Willis aneurysms, interventional neuroradiology and coil insertion should hypothetically be the procedure of choice [64,65,Class IV].…”

“…Indeed, the most frequent localizations of aneurysms in BD are the aorta, the pulmonary arteries, and the femoral arteries. 3 In our patient, the ruptured carotid aneurysm was inaugural and revealed BD. Rupture is an exceptional complication of carotid aneurysms and only some cases were described in the literature.…”

“…In addition, preoperative immunosuppression should be initiated immediately, but postponing the intervention is not always feasible, particularly in symptomatic aneurysms. 2,3,9 In conclusion, patients with BD should be evaluated regularly to diagnose arterial complications early. Extracranial carotid aneurysms are extremely rare in these patients.…”

“…Ligation of the diseased arteries is another treatment choice in appropriate cases because of the high incidence of graft occlusion. 3,6 Owing to the fragility of the involved arteries, patients with vascular Behçet syndrome undergoing surgery are at high risk of late postoperative complications, such as a pseudoaneurysm at the anastomotic site, with a frequency of 30e50%. To prevent this complication, the graft should be anastomosed to a healthy artery.…”
