Beta‐adrenoceptor responses to inhaled salbutamol in the elderly.

Abstract: The purpose of the present study was to evaluate and compare the responsiveness of 32-adrenoceptors in elderly and young subjects. Seven healthy elderly volunteers (72 ± 3 years) were given cumulative doses of inhaled salbutamol (100 ±g-4000 ,ug) or placebo, following pre-treatment with propranolol 40 mg or placebo. Finger tremor (Tr), plasma potassium (K), and heart rate (HR) were measured at each dose step. There were dosedependent increases in Tr (P < 0.001) and HR (P < 0.001) and falls in K (P < 0.001), wh… Show more

“…However, reported data are in favor of the hypothesis that ß 2 -receptor functioning is maintained in the elderly, although one study reported a decrease in the chronotropic cardiovascular response to isoprenaline in healthy elderly subjects [23]. However, two other placebo-controlled studies, evaluating dosedependent increases in finger tremor and heart rate in younger and older subjects to cumulative doses of inhaled salbutamol following pretreatment with propanolol (a ß 1 -and ß 2 -antagonist), showed no evidence for a decrease in ß 2 -adrenoceptor activity in the elderly [24,25].…”

“…Lastly, as beta 2 ‐agonists can increase heart rate and ventilation, cardiac and respiratory muscle work may also contribute to the beta 2 ‐agonist‐induced increase in metabolic rate.…”

“…It was reported that pi-adrenergic receptor responsiveness is reduced with aging,42 whereas p2-receptor responsiveness appeared to remain preserved in elderly compared with young ~u b j e c t s .~~~ 43,44 Although no statistically significant difference in in vivo pulmonary or in vitro T lymphocyte p2-receptor response to terbutaline was seen in elderly over young asthmatics, trends toward a dampened response in vivo and in vitro, as well as reduced cardiovascular pi-receptor response, existed in our elderly subjects. Overall, our in vitro results are consistent with the in vivo observations.…”
