Bilateral Synchronous Testicular Involvement in Multiple Myeloma. Case Report and Review of the Literature

Abstract: The authors describe a case of synchronous bilateral involvement of the testes in a 70-year-old patient seven years after the onset of an IgG k IIIA multiple myeloma. Ultrasonographic and postoperative immunohistochemical studies were performed. A complete review of the literature shows the rarity of testicular plasmacytoma. The present one is the second reported case of syncronous involvement of the testes.

“…2 Castagna et al compiled 57 cases with testicular plasmacytoma and only 11 cases had no multiple myeloma for 2 months to 3 years follow-up periods. 3 The present case was thought to be primary testicular plasmacytoma because there was no evidence of multiple myeloma over 9 months' follow-up. Previous reports of testicular plasmacytoma showed that a painless testicular swelling is the most common chief complaint, although hydrocele of the testis as a chief complaint was found in two cases.…”

“…[ 13 23 ] The timeline for this progression can extend as far as several years and as such, these patients need to be closely monitored given the risk of developing subsequent generalized MM. [ 19 23 24 ]…”

“…Such lesions are most commonly found in patients between ages 55 and 60, often with a known diagnosis of multiple myeloma or at the time of known systemic relapse. [8,10,11] Several cases have been reported where these lesions presented in the absence of such history. [3,10] The patient we present here was above average for age and had previously undergone chemotherapy and autologous bone marrow transplantation for treatment of his known myeloma, so the discovery of his bilateral testicular plasmacytomas was thought to be most consistent with disease recurrence.…”
