Biomechanical and Clinical Evaluation of a Modified 3‐Loop Pulley Suture Pattern for Reattachment of Canine Tendons to Bone

Abstract: Tendon repairs with a gap >3 mm are reported to be at increased risk of rupture during the first 6 weeks postoperatively. A modified 3-loop pulley pattern resists gap formation better than a locking-loop pattern.

“…A modification of these techniques seemed necessary in the present case taking into account that the injury the cat sustained consisted of an avulsion of the triceps tendon from the olecranon and not of a true tendon rupture, leaving the bone and only minimal tendinous material for the surgical repair. A modified three‐loop pulley suture was used for surgical repair since this method has been shown to be superior to a locking loop suture in preventing gap formation during tensile loading of reattached canine gastrocnemius tendons (Moores and others 2004).…”

“…There are a few surgical techniques described in the literature related to the reattachment of the triceps tendon to the olecranon [1, 2, 7, 8, 10]. Presently available options include a tension band fixation, horizontal mattress suture with drilled holes in the proximal olecranon, a modified three-loop pulley suture and a Bunnell-Mayer suture [1, 2, 7, 8, 10].…”

Tension band fixation has been described for the management of a cat with an avulsion fracture of the proximal ulnar epiphysis [7].…”

“…Suture patterns developed for this purpose include the Krakow suture, locking loop, Bun nellMayer and three-loop pulley patterns (Montgomery and Fitch 2003;Moores et al 2004). However, in cases where excessive tension results from retraction of the muscle-tendon unit or extensive tendon damage leading to a significant decrease in tensile strength, tendon repair with suture alone may not be sufficient.…”
