Bruselloz tanısında altın standart kan veya kemik iliği kültüründe mikroorganizmanın üretilmesi olarak kabul edilmektedir. Ancak mikroorganizmayı üretmek her zaman mümkün olmamaktadır. Bu nedenle Bruselloz tanısı için serolojik testler kullanılır. Rose Bengal testi (RB), Standart Tüp Aglütinasyon testi (STA), Coombs Testi (CT) ve 2-Merkaptoetanol (2-ME) testleri yaygın yöntemlerdir. İmmünokaptur-aglütinasyon testi (ICA) ve Brucella Coombs Jel testi (BCGT), toplam antikorlarla birlikte bloke edici antikorları tespit edebilen testlerdir. Çalışmamızın amacı tanıda kullanılan testler ile ICA ve BCGT testleri arasında uygunluk olup olmadığını belirlemektir. Gereç ve Yöntemler: Bruselloz ön tanısı olan hastalardan serum örnekleri alındı. Öncelikle tüm numunelerde RBT testi çalışıldı. Çalışmaya 40 RBT pozitif ve 40 RBT negatif serum dahil edildi. Tüm serum örnekleri STA, CT, 2-ME, ICA ve BCGT yöntemleriyle çalışıldı. Testler arasındaki uyumluluk kappa (κ) katsayısı kohen kappa analiz yöntemi kullanılarak belirlendi.Bulgular: RBT pozitif olan 40 hastanın 28'i STA ve 2-ME ile pozitif, 30'u BT ile pozitif bulundu. BCGT ve ICA test sonuçları tüm RBT pozitif örneklerde pozitif bulundu. Tüm test sonuçları 40 RBT negatif numunede negatif bulunmuştur. Kohen Kappa analizi, RBT ile BCGT ve ICA (Kappa 1.0, p <0.001) arasındaki uyumun mükemmel olduğu bulundu. STA, BCGT ve ICA testleri arasındaki uyumun ise iyi düzeyde olduğu belirlendi (Kappa 0.7, p <0.001). CT testi, ICA ve BCGT arasında çok iyi uyumluluk seviyesi gösterdi (Kappa 0.8, p <0.001). 2ME testi, ICA ve BCGT testleri arasında uyum çok iyi olarak değerlendirildi (kappa 0.7, p <0.001), BCGT ve ICA arasındaki uyumun da çok iyi düzeyde olduğu saptandı (Kappa 1.0, p <0.001). Sonuç: Bu sonuçlara göre ICA ve BCGT testlerinin kullanılmasıyla hem test sayısı azalacak hem de sonuç alma süresi kısalacaktır. Ancak bu testlerin duyarlılığını ve özgüllüğünü kültür sonuçlarına göre belirlemek için daha fazla çalışmaya ihtiyaç vardır. Ancak ondan sonra, teşhis testi olarak kullanılabileceğine inanıyoruz.

Amaç Bruselloz; Brucella türlerinin sebep olduğu zoonotik bir hastalıktır. İnsanlarda gelişen bruselloz tanısında başlıca kültür ve serolojik yöntemler kullanılmaktadır. Biz hastanemizdeki bruselloz serosürveyansını belirlemeyi amaçladık. Materyal ve MetodTemmuz 2014-Temmuz 2019 tarihleri arasında hastanemiz mikrobiyoloji laboratuvarına gönderilen bruselloz ön tanılı hasta örnekleri Rose Bengal testi (Sero-Lam Brusella Rose Bengal Plate Test Seromed, Türkiye) ve Brucella Coombs jel testi (ODAK Brucella Coombs Gel Test, İstanbul, Türkiye) ile incelenmiştir. Rose Bengal testinde ha f ya da belirgin kümelenme görülmesi "pozitif " olarak değerlendirilmiştir. Brucella Coombs jel testinde ise; 1/160 ve üzerindeki titreler pozitif olarak kabul edilmiştir. Bulgular Çalışmada incelenen bruselloz şüpheli 13796 hasta örneğinden 1040 (%7,5) ında Rose Bengal testi pozitif olarak bulunmuştur. Brucella Coombs jel testi pozitif olan hasta sayısı ise 468 (%3,4) dir. 615 (%4,5) serum örneğinde Rose Bengal testi pozitif olup, Brucella Coombs jel testi negatif saptanmıştır. 43 hasta örneği ise Rose Bengal testi ile negatif iken Brucella Coombs jel testi ile pozitif olarak belirlenmiştir. Sonuç Rose Bengal ve Brucella Coombs jel test kullanılarak hastanem zde son 5 yıldak Brusella serosürveyansını araştırdık. Objective: Although brucellosis is the most common zoonosis in the world, it remains an infectious disease that has not received sufficient attention. This study’s aim was to retrospectively examine the results of the Rose Bengal and Brucellacapt tests in serum samples from patients with a prior diagnosis of brucellosis sent to our laboratory in our province, a highly endemic area for brucellosis. Materials and Methods: Between June 2019 and June 2021, 7827 serum samples sent to the public health laboratory with a provisional diagnosis of brucellosis were evaluated. The Rose Bengal and Brucellacapt test (Vircell, Spain) was used diagnose infection. Samples with antibody titer ≥1/160 were considered positive. Results: Of the 7827 samples included in the study, 354 (4.6%) of the total 7677 serum samples tested were RBT positive, 118 (3.1%) of the 3776 samples tested were BCAP positive, and 118 (3.3%) of the 3626 samples tested were both RBT and BCAP positive.
