Cancer in Kashmir, India: Burden and pattern of disease

Aim. The aim of this study was to see the clinical, pathological, and demographic profile of young patients with stomach carcinoma besides association with p53. Patients and Methods. Prospective study of young patients with stomach carcinoma from January 2005 to December 2009. A total of 50 patients with age less than 40 years were studied. Results. Male female ratio was 1 : 1.08 in young patients and 2.5 : 1 in older patients. A positive family history of stomach cancer in the first degree relatives was present in 10% of young patients. Resection was possible only in 50% young patients. 26% young patients underwent only palliative gastrojejunostomy. The most common operation was lower partial gastrectomy in 68%. Amongst the intraoperative findings peritoneal metastasis was seen in 17.4% in young patients. 50% young patients presented in stage IV as per AJCC classification (P value .004; sig.). None of the patients presented as stage 1 disease in young group. Conclusion. Early detection of stomach carcinoma is very important in all patients but in young patients it is of paramount importance.

Background: Ovary is a complex structure and its neoplasms show a wide spectrum of histological types and clinical behaviour. The present study was done with the aim of studying the histopathological pattern of the ovarian tumours in women of various age groups.Methods: This was prospective study conducted over a period of 2 years from Jan 2015 to Dec 2016 and included 193 cases of ovarian neoplasms.Results: Of 193 cases 155 (80.3%) were benign, 8 (4.1%) were borderline and 30 (15.5%) were malignant tumours. Surface epithelial tumours constituted majority of the tumours (64.7%). Mature cystic teratoma (30.6%%) was the most common benign tumour followed by serous cyst adenoma (26.9%). Serous cyst adenocarcinoma (7.8%) was the most common malignant tumour of the ovary. While tumours were seen over a wide range of 6-75 years, >80% of benign tumours were seen in patients of <40 years and 60% of malignant tumours were seen in patients of >40 years of age.Conclusions: The findings of present study are comparable to the published data from the subcontinent.

Salt tea consumption and esophageal cancer: A possible role of alkaline beverages in esophageal carcinogenesis

Salt tea is the most commonly used beverage in Kashmir, India, where esophageal squamous cell carcinoma (ESCC) is the most common cancer. Salt tea is brewed in a unique way in Kashmir, usually with addition of sodium bicarbonate, which makes salt tea alkaline. As little information about the association between salt tea drinking and ESCC was available, we conducted a large-scale case-control study to investigate this association in Kashmir. We recruited 703 histologically confirmed cases of ESCC and 1664 controls individually matched to cases for age, sex, and district of residence. Conditional logistic regression models were used to calculate odds ratios (ORs) and 95% confidence intervals (95% CIs). Participants who consumed >1,250 ml day 21 showed an increased risk of ESCC (OR 5 2.60, 95% CIs 5 1.68-4.02). Samovar (a special vessel for the beverage preparation) users (OR 5 1.77, 95% CIs 1.25-2.50) and those who ate cereal paste with salt tea (OR 5 2.14, 95% CIs 5 1.55-2.94) or added bicarbonate sodium to salt tea (OR 5 2.12, 95% CIs 5 1.33-3.39) were at higher risk of ESCC than others. When analysis was limited to alkaline tea drinkers only, those who both consumed cereal paste with salt tea and used samovar vessel were at the highest risk (OR 5 4.58, 95% CIs 5 2.04-10.28). This study shows significant associations of salt tea drinking and some related habits with ESCC risk.

Abstract: Pattern of malignancies in valley is different from rest of the India. Gastrointestinal malignancies are the commonest malignancies in Kashmir valley followed by lung cancer in males and breast cancer in females.
