Capacidade funcional em indivíduos com paraparesia espástica tropical/mielopatia associada ao HTLV-1

Objetivo: Caracterizar e correlacionar aspectos da qualidade de vida e funcionalidade, em indivíduos portadores da Mielopatia (PET-MAH) associada ao HTLV-1. Metodologia: Estudo transversal, observacional, descritivo e analítico com coleta de dados primários. Participaram desta pesquisa 15 voluntários, sendo 8 do sexo feminino e 7 do sexo masculino no ambulatório de cardiorrespiratória da UEAFTO/UEPA com portadores do vírus HTLV-1. Foram utilizados questionários de domínio público e validados na língua portuguesa. Na avaliação da Qualidade de vida foi utilizado a versão brasileira do questionário de qualidade de vida SF-36 e para análise da funcionalidade foi utilizado o índice de Barthel. Resultados: Os principais resultados obtidos através do Questionário SF-36 demonstraram redução da qualidade de vida nos aspectos de capacidade funcional (16.66 ± 27.86 / 66.11 ± 35.15, p-valor 0.013) e aspectos sociais (33.16 ± 18.77 / 73.77 ± 25.96, p-valor 0.006) do G2 em relação ao G1. Já os resultados com o Índice de Barthel apresentaram indivíduos classificados com dependência parcial no grupo G1 para aspectos como “Vestir-se”, “Urinário” e “Escadas” e no grupo G2 indivíduos com dependência em todos os aspectos, exceto “Intestino”. Conclusão: O estudo revelou pontos que podem ser afetados na QV de vida dessa população, afetando principalmente a capacidade funcional e aspectos sociais em indivíduos com sintomas clínicos mais avançados. Visto isso, a identificação, avaliação funcional e de QV precoce de indivíduos portadores do vírus HTLV-1 para a identificação de déficits motores e aspectos individuais é essencial para evitar a diminuição da qualidade de vida desses pacientes.

IntroductionHuman T-lymphotropic virus 1 (HTLV-1)-associated myelopathy (HAM) restricts activities of daily living (ADLs), affecting health and quality of life. Occupational therapy is used to promote independence during ADL in people living with HTLV (PLHTLV).ObjectiveTo quantify the clinical aspects, levels of functionality, performance in ADLs and occupational roles of PLHTLV and propose an occupational therapeutic intervention.MethodA cross-sectional, descriptive, observational study was designed with 40 PLHTLV monitored at two referral laboratories of the Federal University of Pará. The Evandro Chagas Research Institute Neurological Disability Scale (EIPEC-2), the Barthel Index and the Occupational Roles Identification List were applied. The G test and Fisher’s exact test (to identify associations between qualitative variables), the Mann–Whitney test (to identify associations between quantitative variables) and Pearson correlation analysis (to identify associations between the total Barthel Index and EIPEC-2 scores) were performed with Microsoft Excel and BioEstat 5.0, and the significance level was set at p ≤ 0.05.ResultsMotor aspects, the presence of spasticity, sensory aspects, and pain in the lumbar region and lower limbs were significantly (p = 0.0002) higher among symptomatic individuals, who also had more difficulties urinating, traveling up and down stairs and transferring from a chair to a bed. Being a worker, performing housework and socializing (with friends or family members) were the most affected activities among HAM patients.ConclusionThe impact of HAM on PLHTLV should include an intervention plan with occupational therapists in rehabilitation programs to create an important third-level prevention initiative, which may help achieve short-, medium-, and long-term goals. Asymptomatic PLHTLV should also be able to prevent future ADL impairment.

Introduction: The presence of human T-cell lymphotropic virus type 1 (HTLV-1) associated with neuropathy (myelopathy/tropical spastic paraparesis - HAM/TSP), can generate morphological and functional changes in the respiratory system. As a preventive therapeutic possibility for respiratory dysfunctions, it is expected that the already conceptualized inspiratory muscle training, when performed at home, can be a therapeutic resource that favors adherence to treatment. Objective: To evaluate respiratory muscle strength in patients with HTLV-1 after participating in a home respiratory muscle training protocol under indirect supervision. Method: This was a clinical, longitudinal, prospective, quantitative, and single-center trial approved by the Research Ethics Committee of the State University of Pará, opinion no. 2.695.505 and registered in clinical trials NCT03829709. Six HTLV-1 patients participated in a 5-week home respiratory muscle training protocol lasting 30 minutes daily through a linear load inspiratory muscle trainer. For the characterization of the imposed load, they were submitted to manovacuometry during pre (T0), peri (T3), and post (T5) treatment. Results: Six individuals completed the program, of which 83.33% were female and 16.66% male. With the application of respiratory muscle training, it was possible to achieve a significant increase (p < 0.011) of the maximum inspiratory pressure as shown when comparing T0 (66.8±12.58) to T5 (115.08±31.78). Conclusion: This study identified an increase in inspiratory muscle strength after HTLV-1 patients participated in a home muscle training protocol under indirect supervision.
