Cardiac Angiosarcoma Diagnosed by Transvenous Endomyocardial Biopsy With the Aid of Transesophageal Echocardiography and Intra-Procedural Consultation

Abstract: SummaryWe report a case who had confirmed tumor cells in the biopsy specimens by transvenous endomyocardial biopsy with intra-procedural consultation and fast smear cytology. A 57-year-old female was admitted to our hospital because of shortness of breath and left back pain. Transthoracic echocardiography (TTE) and contrast-enhanced computed tomography (CT) scans demonstrated a large mass in the right atrium and multiple liver tumors thought to be due to spread of the disease. Coronary angiography showed the r… Show more

“…All angiosarcomas in our group arose from the right atrium, with two extending into the tricuspid valve. The tumor was large at the time of presentation, sometimes as large as 9 x 7 x 5 cm, consistent with a review of angio-sarcomas, 34) Two patients' samples were positive for classic markers of angiosarcoma: 35) CD31 and CD34. Weibel-Palade bodies were found in another patient's tissue by electron microscope.…”

“…In the case presented here, the patient had hemoptysis together with hemopneumothorax and pleural infiltration. A small number of cases have been reported to present with thin-walled pulmonary cysts that developed and enlarged within a few months [4]. The role of positron emission tomography (PET)-CT in evaluation of distal metastasis of AS is not clear.…”

“…Chest CT is useful to identify metastasis. Cytological analysis of pericardial effusion is also helpful; with the positive rate ranging from 75% to 87%.A transvenous endocardial biopsy procedure was reported in one study 8 . Despite all the above methods, surgical biopsy is always necessary to yield an adequate sample for diagnosis.…”
