Cardiac CT Assessment of Prosthetic Aortic Valve Dysfunction Secondary to Acute Thrombosis and Response to Thrombolysis

Abstract: A 58-year-old woman presented with a 2-week history of acute dyspnea in the setting of subtherapeutic warfarin anticoagulation (international normalized ratio, 1.7 on admission) with a history of previous St. Jude bileaflet aortic valve replacement (AVR) 5 years previously. Transthoracic echocardiography revealed severe elevation of the prosthetic aortic transvalvular gradient (peak velocity, 5 m/s; peak gradient, 125 mm Hg) with severe eccentric aortic regurgitation. The morphology of the prosthetic aortic va… Show more

“… A strong asymmetry in the shear stress distribution was observed. A relevant clinical implication can be seen in that result, which can possibly explain the asymmetric distribution of pannus/thrombotic deposits that is sometimes reported for explanted BMHV [ 38 , 63 ]. Whether local flow dynamics asymmetry is related to the asynchronous behaviour of valve leaflets and/or vice versa, and the latter to valve implant orientation with respect to the sinuses of Valsalva, deserves further investigation.…”

“…16,17 Preliminary reports indicate the potential additive clinical value of multidetector computed tomography (MDCT) to standard care for the diagnosis of prosthetic valve (including THV) thrombosis. 8,9,18,19 Given the limitations of routine TTE thrombosis surveillance of postimplant THVs, we sought to determine the incidence of THV thrombus on post-TAVI MDCT and to determine potential causative factors using a multimodality imaging strategy comprising TTE, TOE, and MDCT.…”
