Cardiac effects following adjuvant chemotherapy and breast irradiation in operable breast cancer

Abstract: The addition of full-dose adriamycin to alkylating-containing adjuvant chemotherapy, as given in our studies, failed per se to increase the frequency of cardiac effects. Thus anthracyclines, which have the potential to improve current treatment results, deserve a proper place in the design of future adjuvant studies.

“…Patients were followed for a median of 80 months. In the patients treated with doxorubicin, no cases of CHF were reported in patients who had underwent right breast RT, however, 3 of the 114 who had received doxorubicin and left breast RT had CHF within 1 month after completing adjuvant treatment [72]. Late cardiac toxicity with anthracyclines and RT may also manifest after several years.…”

“…The risk of cardiotoxicity is also increased for patients treated with anthracyclines and chest radiotherapy. In breast cancer patients, the use of ‡450 mg/m 2 doxorubicin, or the concurrent use of anthracycline and local radiation in left-sided breast cancers as compared to rightsided breast cancers led to significant higher rate of cardiac heart failure (Valagussa et al, 1994;Landier et al, 2004). Additional risk factors of late anthracycline toxicity are male gender in adults and female gender in children, older age, associated cardiac risk factors and baseline cardiac disease (Hequet et al, 2004).…”

“…The majority of the trials are relatively small or the trial designs are such that conclusions cannot be drawn. Various aspects of cardiac morbidity are addressed in four trials (34,35,38,41). The literature shows that:…”
