Aims: Evidence for CRT in adults with congenital heart disease (ACHD) and chronic heart failure is limited, with recommendations for its use extrapolated from the population with structurally normal hearts. This retrospective observational study investigates the efficacy of CRT in this heterogenous group, discussing factors predicting response to CRT.Methods: Twenty-seven patients with structural ACHD who underwent CRT insertion or upgrade at a tertiary center in the United Kingdom were retrospectively studied.The primary outcome measure was clinical response to CRT, defined as improvement of NYHA class and/or improvement in systemic ventricular ejection fraction by one category. Secondary outcomes included change in QRS duration and adverse events.Results: Thirty-seven percent of patients had a systemic right ventricle (sRV). RBBB was the commonest baseline QRS morphology (40.7%) despite this being an unfavorable characteristic for CRT. Overall, positive response to CRT was demonstrated in 18 patients (66.7%). NYHA class improved in 55.5% following CRT (p = .001) and 40.7% showed improvement in systemic ventricular ejection fraction (p = .118). There were no baseline characteristics that predicted response to CRT, and electrocardiographic measures such as QRS shortening post-CRT was not associated with positive response.Good response rates (60.0%) were demonstrated in those with sRV. Conclusion:CRT is efficacious in structural ACHD including in those who do not meet conventional criteria. Extrapolation of recommendations from adults with structurally normal hearts may be inappropriate. Future research should focus on improving patient selection for CRT, for example using techniques to better quantify mechanical dysynchrony and intra-procedural electrical activation mapping in these complex patients.

Obesity with dyslipidemia is a serious problem because it risks various degenerative and metabolic diseases. The prevalence of obesity in Indonesia continues to increase every year, currently in the adult population reaching 21.8%. To prevent the onset of degenerative diseases and metabolic disorders, it is important to control obesity with dyslipidemia through weight loss and lipid profile improvement. This study aims to determine the effect of nutritional counseling in losing weight and improving lipid profiles in obese patients with dyslipidemia. This is a pseudo-experimental study with One group Pre and Post Test Design. The initial stage was the identification of obesity and lipid profile. In 100 respondents identified as obese, total cholesterol, LDL and triglycerides were examined. 40 obese respondents with dyslipidemia were determined as samples. The second stage was nutrition counseling intervention. The third stage of evaluation is the re-measurement of body weight and lipid profile. The research result is only 30 respondents participated in the evaluation. Average initial body weight was 75.11 kg and final body weight was 73.14 kg. Average weight loss was 1.97 kg. Average initial cholesterol 233.20 mg/dL and final cholesterol 224.70 mg/dL. Average cholesterol reduction of 8.5 mg/dL. Average initial LDL 152.37 mg/dL and final LDL 139.47 mg/dL. Average decrease in LDL 12.9 mg/dL Average initial triglycerides 161.23 mg/dL and final triglycerides 143.97 mg/dL. The average decrease in triglycerides is 17.26 mg/dL. The statistical test results showed a p-value = 0.000. There is an effect of nutritional counseling on weight loss and improvement of lipid profiles of obese patients with dyslipidemia.

Penyakit jantung penyebab kematian nomor satu pada wanita. Tetapi kenyataannya hanya sedikit wanita yang mengetahui faktor risiko penyakit jantung. Metode yang digunakan studi literatur dengan mencari artikel di situs ilmiah seperti ScienceDirect, Pubmed, Elsiver, NCBI, DOAJ, Proquest, Microsoft Academic, Sage Journal, Oxford Academic, LIPI, ResearceGate, JSTOR dan Scrib.id s menggunakan kata kunci women dan heart disease sehingga didapatkan 37 artikel dari tahun 2018 sampai dengan tahun 2022 .Tujuan dari penelitian ini mengetahui faktor risiko yang menyebabkan wanita mengalami penyakit jantung dan hambatan menerapkan perilaku jantung sehat pada wanita. Hasilnya wanita mengalami penyakit jantung ketika mereka remaja mempunyai persepsi kalau penyakit jantung hanya dialami oleh orang tua dan beransumsi penyakit jantung banyak diderita oleh pria. Selain faktor tersebut norma sosial juga menjadi penyebab penyakit jantung pada wanita karena wanita lebih fokus pada berat badan. Faktor yang menjadi hambatan wanita dalam menerapkan perilaku jantung sehat antara lain stress dan kurangnya waktu. Karena wanita akan menyadari mereka beresiko mengalami penyakit jantung pada dirinya jika jantung ada masalah sejak awal. Kesimpulannya berat badan, norma sosial dan persepsi tentang penyakit jantung berkontribusi pada rendahnya kesadaran faktor risiko penyakit jantung di kalangan remaja putri sehingga membatasi untuk menerapkan perilaku jantung sehat. The method used is a literature study searching for 37 articles from 2018 to 2022 on ScienceDirect, Pubmed, Elsevier, NCBI, DOAJ, Proquest, Microsoft Academic, Sage Journal, Oxford Academic, LIPI, ResearchGate, JSTOR, and Scrib use keywords women heart disease. This study aims to determine the risk factors that cause women to experience heart disease and inhibit healthy heart behaviour in women. The result is that women who experience heart disease when they are teenagers perceive that parents only experience it and assume that men predominantly suffer heart disease. In addition to these factors, social norms are also a cause of heart disease because women are more focused on weight. Factors that hinder women in implementing healthy heart behaviours include stress and lack of time. Because women will realize they are at risk for heart disease in themselves if the heart has problems from the start. In conclusion, weight, social norms and perceptions of heart disease contribute to the low awareness of heart disease risk factors among adolescent girls, limiting them to adopt healthy heart behaviours.
