Carotid Artery Atherosclerosis: In Vivo Morphologic Characterization with Gadolinium-enhanced Double-oblique MR Imaging—Initial Results

Abstract: In nine subjects with carotid atherosclerosis, double-oblique, contrast material-enhanced, double inversion-recovery, fast spin-echo magnetic resonance (MR) images were acquired through atheroma in the proximal internal carotid artery. Fibrocellular tissue within atheroma selectively enhanced 29% after administration of gadolinium-based contrast agent. Contrast enhancement helped discriminate fibrous cap from lipid core with a contrast-to-noise ratio as good as or better than that with T2-weighted MR images bu… Show more

“…Previous work has shown excellent contrast between the cap and core with imaging at 5 minutes following injection (13). There does not appear to be a gain in CNR beyond the first 10 minutes following gadolinium administration.…”

“…The cap was identified as an area of relative hyperintensity adjacent to the lumen, and the lipid core as an area hypointense and deep to the cap on T2-weighed images. The delineation of the cap and core was confirmed using the T1-weighted postcontrast DIR FSE sequence when acquired, with the cap appearing hyperintense relative to the core (13). The ROIs were copied from these images onto the IR FSE images (Fig.…”

These parameters may be extracted from a DCE-MRI time course via the application of models that describe the distribution of contrast agent in vivo.…”

“…In past decades, there has been considerable progress in MR scanning technology and parameters, including the development of multicontrast MR protocols 73, 74, 75. Furthermore, image acquisition time, success rate, the demonstration of calcification, and the diagnostic accuracy of cardiac magnetic resonance angiography have all steadily improved 76, 77, 78, 79, 80…”
