Case Report of Urinary Schistosomiasis in a Returned Traveler in Korea

Abstract: A 23-year-old Korean woman with a residence history in Kenya and Malawi for about 2 years presented with gross hematuria for 1 month. Blood tests were within normal range except eosinophilia. Asymmetrically diffuse wall thickening and calcification were observed at the urinary bladder on CT. Multiple erythematous nodular lesions were observed in the cystoscopy and transurethral resection was done. Numerous eggs of Schistosoma haematobium with granulomatous inflammation were observed in the submucosal layer of … Show more

“…Serology is used erratically in the diagnosis of patients with cUGS-related manifestations. In several reports [ n = 7], patients underwent invasive procedures and were also found to have a positive serology for S. haematobium [ 85 , 91 , 92 , 105 , 137 , 139 , 149 ]. On the contrary, in several reports [ n = 11], eggs were not present in the urine of patients with chronic schistosomiasis [ 92 , 98 , 100 , 102 , 103 , 106 , 126 , 134 , 137 , 139 , 140 ].…”

“…A definitive diagnosis of schistosomiasis requires histopathology, which reveals the characteristic S. haematobium eggs with their terminal spines and granulomas. The current understanding is that S. haematobium is the cause of GU schistosomiasis, while other Schistosoma species cause intestinal schistosomiasis[ 1 , 9 , 11 , 12 ]. However, our patient showed the presence of eggs with a lateral spine with GU schistosomiasis, which could indicate the possibility of an infection with another Schistosoma species.…”

“… 2 Schistosomiasis is not endemic in Korea and therefore, only the imported cases have been reported in travelers to endemic areas of schistosomiasis. 3 The strategy for schistosomiasis control aims to prevent morbidity in later life through regular treatment with praziquantel, which is currently the only recommended drug for curing schistosome infections. 4 5 A number of international agencies, notably the United Nations International Children's Fund and the World Health Organization (WHO), 6 along with for-profit and non-profit organizations, supply praziquantel in bulk to developing countries.…”
