Case Report on Leiomyosarcoma of the Vulva: A Rare Pathology

Abstract: Leiomyosarcoma (LMS) of the vulva is rare. However it is the most common histologic subtype of vulvar sarcoma, accounting for approximately 1% of all vulvar malignancies. 1-8 Whether genetics and epigenetics play a role in pathogenesis is unclear. 1 The tumor is slow-growing with non-specific symptoms, has high metastatic potential, and follows a bimodal age distribution. 1-8 Diagnosis and prognosis are based upon immunohistochemical expression and criteria from early literature. 1,3,5-7 The most common therap… Show more

“…Of the 26 cases of VLMS reported in the literature, 6 patients underwent a LND, although imaging suggested possible lymph node metastases in the bilateral external iliac lymph nodes in only 1 case ( Table 3 ) [ 36 ]. In 5 of these 6 cases, an inguinal LND was performed, and in 1 case an inguino-femoral LND was performed [ 43 ]. The inguino-femoral LND was done bilaterally, and 4 of the 5 inguinal LND were done ipsilaterally.…”

“…Information on the side of the LND was missing in 1 case [ 51 ]. No lymph node metastases were detected in any of these 6 cases [ 33 , 36 , 43 , 45 , 49 , 51 ]. Information on follow-up was provided for 4 of the 6 patients with LND [ 36 , 45 , 49 , 51 ].…”

“…Additional measures may include hemivulvectomy, ipsilateral inguinal lymphadenectomy radiation, or chemotherapy at the discretion of the oncologist. The necessity of adjuvant therapy is unknown currently [5,7,9,16]. The local recurrence and frequency of metastasis by the hematogenous route are high with a very aggressive nature and poor prognosis, However, the course of the disease and late recurrence are still not clear [11].…”
