Cerebrospinal fluid analysis and the determination of oligoclonal bands

Abstract: This document presents the guidelines for the cerebrospinal fluid (CSF) analysis and the determination of oligoclonal bands (OCBs) as pivotal tests in neuroin flammatory pathologies of the central nervous system. The guidelines have been developed following a consensus process built on questionnaire-based surveys, internet contacts, and discussions at workshops of the sponsoring Italian Association of Neuroimmunology (AINI) congresses. Essential clinical information on the pathologies in which the CSF analysis… Show more

“…Luminescent detection, using either fluorescence or chemiluminescence, is routinely used after electrophoretic separation of proteins, mainly after sodium dodecyl sulfate (SDS) electrophoresis 11 Although chemiluminescence has been mentioned as one of the recommended techniques for IgG immunodetection in recently published guidelines on CSF analysis 12 , we were able to find few reports on luminescent detection of proteins separated by IEF as antigen-specific oligoclonal IgG (ref. [13][14][15] ) or oligoclonal IgM (ref.…”

“…In 2017, cerebrospinal fluid (CSF) is done to look for oligoclonal band and immunoglobulin G (IgG), and a parallel serum sample need to be taken as no OCB production is noted in the blood in multiple sclerosis. Oligoclonal band and immunoglobulin G are indicative of intrathecal inflammation which is B cell modulated from plasma cells seen in CNS inflammatory disease [20]. Distinctive CSF analysis will disclose slightly raised leucocyte count, B cells, or plasma cells in cytological analysis and raised IgG synthesis [21].…”

“…18 According to guidelines, a positive OCB test is defined by the presence of at least two CSFspecific bands. 19 Therefore, mixed patterns were considered to be positive when meeting this criterion. On the other hand, mirror patterns, sometimes encountered in systemic inflammatory diseases, were considered to be negative.…”
