Cerebrospinal fluid fistula in a patient with chronic constipation related to an autonomic dysfunction and revealed by bacterial meningitis – A case report

Abstract: Background: Cerebrospinal fluid (CSF) fistula represents a rare neurosurgical entity that can be defined as a communication between the subarachnoid space and nasal fossa or less commonly the ear cavity. It can be spontaneous without an evident etiology or secondary following a skull base surgery or trauma. The early diagnosis of spontaneous forms remains a challenge as clinical signs (e.g., unilateral rhinorrhea) can be absent or neglected by patients and can result in meningitis. “…MR cisternography and MR myelography are often used for the diagnosis and neurosurgical treatment of anatomical CSF disorders ( Mokri, 2014 ). Rhinorrhea, defined as the leakage of CSF from the subarachnoid space into the nasal fossa, is a common reason for MR cisternography ( Abboud et al, 2020 ). Extending beyond leaks, CSF clearance pathways may have direct relevance to Alzheimer’s disease and possibly to other neurodegenerative diseases ( Tarasoff-Conway et al, 2015 ; Mehta et al, 2022 ; Kylkilahti et al, 2021 ).…”

“…Depending on their etiopathogenesis, effusions can be subdivided into being caused by malignancy, congestive heart failure, renal and liver failure, infection, and effusions due to pulmonary embolism. Despite being less common, attention should also be given to traumatic and tuberculous pleural effusions, effusions due to serositis, and effusions due to pancreaticopleural and cerebrospinal fluid (CSF) fistulae [ 3 ].…”

“…Spontaneous cerebrospinal fluid (sCSF) leaks of the temporal bone occur when a defect over a pneumatized space results in encephalocele, dural defect, and fistula from the subarachnoid space to a pneumatized space such as the mastoid cavity or middle ear (1). sCSF leaks are associated with female sex, obesity, and age of 45-65 years (2)(3)(4)(5).…”
