Cerebrovascular Events in Patients With Significant Stenosis of the Carotid Artery Are Associated With Hyperhomocysteinemia and Platelet Antigen-1 (Leu33Pro) Polymorphism

Abstract: Background and Purpose-Although risk factors for carotid artery stenosis caused by atherosclerosis are known, it is unclear what triggers "activation" of the atherosclerotic plaques and the ensuing thromboembolic cerebral events. The aim of this study was to evaluate whether thrombophilic factors, platelet glycoprotein (GP) polymorphisms, and homocysteine are associated with a risk of ischemic events in patients with significant carotid stenosis. Methods-Consecutive patients with Ն50% carotid stenosis, whether… Show more

“…Preoperative carotid artery duplex scanning was performed to assess the severity of the carotid stenosis, which was quantified as present (stenosis [50%) or absent (stenosis \ 50%). 21 Transesophageal echocardiography was performed in all patients to evaluate the severity of aortic atherosclerosis (using the Katz score). 22 Delirium assessment All patients were assessed for delirium six to 24 hr after extubation by a nurse trained to use the delirium rating scale R98 (DRS-R98).…”

“…This study was therefore excluded from the analysis. 22 Among the remaining 12 studies, 7 studies focused on Thr/Met (rs6065) polymorphism, 7,20,21,[23][24][25][26] , 3 studies examined Kozak sequence Ϫ5 T/C (rs2243093) polymorphism, 27-29 and 2 studies examined both polymorphisms 30,31 (see Table 1). The more recent (2001) Sonoda paper 31 used the same data as their earlier paper (2000) with slightly more cases 23 to conduct a haplotype analysis on Thr/Met and Kozak sequence Ϫ5T/C polymorphism; hence, our meta-analysis used the 2001 study for both Kozak sequence Ϫ5T/C and Thr/Met polymorphisms.…”

“…Streifler e cols. 9 utilizaram 153 pacientes com estenose de carótida extracraniana > 50%, avaliaram seus níveis de homocisteína e observaram que aqueles com hiperhomocisteinemia possuíam um risco quatro vezes maior de desenvolver eventos isquêmicos cerebrais (OR: 4,07; IC de 95%) do que aqueles com níveis normais de homocisteína.…”
