Cesarean scar pregnancy: a systematic review of treatment studies

Abstract: This review recommends treatment options for CSP in clinical practice, based on efficacy and safety. The literature supports an interventional rather than medical approach. Present recommendations are primarily based on case series. Multicenter, well-designed studies are needed to draw definite conclusions on how to treat CSP.

“…Birch Petersen et al reviewed 2037 CSP cases, noted complications in each treatment, and recommended five approaches: resection through a transvaginal approach, laparoscopy, uterine artery embolization (UAE) in combination with dilatation and curettage (D&C) and hysteroscopy, UAE in combination with D&C [3], and hysteroscopy. However, transvaginal sono-guided local methotrexate (MTX) injection was not recommended because of observed major complications and the need for additional systemic treatments [4].…”

“…But, systemic methotrexate appears to be more effective in women with B-HCG level <5000 mU/mL according to the literature. 13,16 In our study all the patients had B-HCG levels <5000 mU/mI. In only 2 patients multidose (2 doses) were required.…”

“…Curettage after UAE is currently accepted as an effective treatment for CSP. UAE can accurately detect and embolize the uterine arteries for efficient reduction of bleeding risk during curettage [2, 17, 18]. However, UAE requires digital imaging equipment and such equipment are expensive.…”
