Changes in plasma cholesterol and triglyceride levels after treatment with oral isotretinoin. A prospective study

Abstract: Twenty men with nodulocystic acne were treated with oral isotretinoin (13-cis-retinoic acid) for four months. Plasma lipids and lipoprotein determinations were obtained before and during treatment to quantitate the effects of oral isotretinoin on lipid metabolism. Maximum isotretinoin-induced elevations in plasma triglyceride and cholesterol levels were 67% and 16%, respectively. Additional maximal changes included very-low-density lipoprotein cholesterol increases of 56%, low-density lipoprotein cholesterol i… Show more

“…Retinoid therapy is known to be associated with hypertriglyceridemia and often with low HDL-cholesterol (13,14). Significant alterations in serum triglycerides levels were not detected in this series, likely due to the low doses of acitretin used.…”

“…One of these side effects is increased cholesterol levels (Lyons et al, 1982;Zech et al, 1983;Marsden et al, 1984;Bershad et al, 1985;Michaelsson et al, 1986;Barth et al, 1993;Fex et al, 1996;Ostlere et al, 1996;Lestringant et al, 1997). Here, the effects of 13-cis-RA on serum chemistry were studied after oral treatment with doses that mimic plasma levels achieved by humans taking the recommended doses.…”

“…Here, neither 13-cis-RA nor alltrans-RA altered overall levels of cholesterol, a fineding similar to that reported by Gerber and Erdman (1980) after 7 days of dietary exposure to 315 µg RA g −1 diet of 13-cis-RA or all-trans-RA. Longer durations of exposure may be necessary for cholesterol elevations (Lyons et al, 1982;Zech et al, 1983;Bershad et al, 1985;Michaelsson et al, 1986;Barth et al, 1993;Ostlere et al, 1996;Lestringant et al, 1997).…”

“…Synthetic retinoids have been well documented to affect the lipid profile, causing elevations in both triglycerides and cholesterol (Zech et al, 1983;Bershad et al, 1985;Vahlquist et al, 1985;Marsden, 1986;Tangrea et al, 1993). Data for high dose vitamin A (300 000 IU per day) show a similar effect on both triglycerides and cholesterol (Murray et al, 1983;Infante et al, 1991;Pastorino et al, 1991).…”
