Changes of OPG and RANKL concentrations in Crohn's disease after infliximab therapy

Abstract: Background: Osteoporosis is a well‐known complication of Crohn's disease (CD). Osteoprotegerin (OPG) concentration is elevated in patients with CD compared to healthy controls. Long‐term infliximab (IFX) maintenance therapy improves the patients' bone mineral density. The effect of IFX on bone metabolism has not yet been clarified. Our aim was to evaluate IFX effects on bone pathology in CD patients. Methods: Twenty‐nine patients were treated with IFX as an induction therapy according to international guidelin… Show more

“…It increases the activity and prolongs the lifetime of osteoclasts by decreasing apoptosis. RANKL is a mediator of the formation, function and survival of osteoclasts, and it is considered a main mediator of bone resorption 31,32 . BSP is produced by osteoblasts, osteoclasts, osteocytes and hypertrophic chondrocytes 33 .…”

“…This pathway has been under intense scrutiny for the development of therapeutic approaches to prevent osteopenia and osteoporosis in postmenopausal women, in primary hyperparathyroidism, Paget's disease, rheumatoid arthritis, periodontal bone loss, and several other disorders (85). RANKL/OPG system is known to be activated in IBD patients (13,72) and can be normalized with infliximab (69). Exogenous recombinant OPG (OPG-Fc) reverses skeletal abnormalities and reduces colitis in IL-2 knockout mice (10).…”
