Characteristic findings of cervical Papanicolaou tests from transgender patients on androgen therapy: Challenges in detecting dysplasia

Abstract: The evaluation of dysplasia can be challenging on Pap tests from transgender patients on androgen therapy. The cohort evaluated had higher rates of unsatisfactory and abnormal Pap tests. Pathologists should be familiar with the distinctive cytomorphological changes in the Pap tests from patients on androgen therapy to evaluate them appropriately.

“…In another US study, 3 64.3% of TM were up to date with screening recommendations as opposed to 73.5% of cisgender women. Additionally, TM have more inadequate Pap test results due to cytomorphological changes (atrophy) associated with androgen therapy 20,21 . The androgenic effect on genital tract, with tissue epithelial atrophy and tissue shrinkage, results in speculum insertion and cervical screening test collection being more uncomfortable and indeed painful.…”

“…Intervention research shows that training on Lesbian, Gay, Bisexual and Transgender+ (LGBT+, ‘with the “+” indicating inclusion of all sexual and gender minority identities’) topics as short as half a day for fifth‐year undergraduate students improved their confidence clinically to assess a transgender patient from 35 to 84% 25 . Cytologists need to be aware of the particular features of dysplasia in atrophic cervical cells, to avoid over‐ and under‐diagnosis 21 …”

“…Despite the recommendations for screening in TM, studies indicate that compared to cisgender women, TM report the following: higher rates of abnormal CCS results (Adkins et al, 2018), higher likelihood of not receiving CCS in their lifetime (37% TM vs. 10% cisgender women; Rahman et al, 2019), and lower likelihood of receiving regular CCS (56% TM vs. 72% cisgender women, p = .001; Kiran et al, 2019). Statistically significant discrepancies have been reported in screening status based on gender identities (p = .041).…”
