“…Symptoms at the time of diagnosis and presence of several distinct electrocardiogram findings (T‐wave repolarization disturbances, atrial fibrillation, right bundle branch block, prolonged PR interval) are predictive for an increased incidence of adverse events during follow‐up. While electrophysiological study (EPS) was only performed in a minority of cases, 10 patients with preoperatively inducible VT were free from arrhythmia at the second EPS after surgical correction 2 . However, in our first patient presenting initially with sudden death by ventricular arrhythmia, the preserved inducibility of VT during electrophysiological testing indicated imperatively the need for ICD implantation, despite complete resection of the diverticulum.…”

“…The development of an LVD can be explained by an interruption during the development of the embryonic ventricle in this period. 2 LVD has a narrower inlet, and layers of myocardium and contractility synchronized to cardiac cycle, which is different to LV aneurysm (LVA). Although we did not obtain any tissue samples, this lesion was very similar to an LVD because the lesion had a narrow neck and synchronized myocardial contraction.…”

“…The mechanism of the in situ relapse of the LVD in our case is unknown, though the fracture of the suture should be the most supposed reason [1]. We prefer a new minimally invasive technique, transapical transcatheter LVD device closure, rather than conventional re-open heart surgery in this case for several reasons.…”
