Chemotherapy-induced lactose intolerance in adults

Abstract: Background. Anorexia and weight loss contribute to the morbidity and mortality from cancer. This study was designed to test the hypothesis that chemotherapy produces lactose intolerance which could have an adverse effect on the nutritional status of patients receiving cytotoxic drugs. Methods. Twenty‐seven patients were evaluated for the development of lactose intolerance during chemotherapy. Lactose breath hydrogen testing (LBHT) was used to assess lactose malabsorption objectively. This test is based on the … Show more

“…New-onset lactose intolerance is the cause of diarrhoea and bloating during 5-fluorouracil chemotherapy in 10% of patients 14 15. Clinical experience suggests that small bowel bacterial overgrowth, bile acid malabsorption and pancreatic insufficiency are three other important contributory factors to chemotherapy-induced GI symptoms 16…”

“…Thus, a negative HBT may be due to a shift from hydrogen to methanogen-producing bacteria. The lactose-HBT has been previously applied in patients, both children 50 and adults, 51 with varying results, but overall it non-Caucasians exhibit an age-related low lactase activity, 46 indicating that the 13 C-lactose breath test is not a suitable marker of small intestinal damage. In comparison, sucrase levels in the brushborder remain relatively constant throughout life, 65 and only 0.2% of the population present with a genetic sucrase deficiency.…”
