Objective: To identified the relationship between general and spinal Anesthesia upon breastfeeding and (demographic &reproductive) : Comparative Study. Methodology: The present study employs a descriptive comparative design held at the labor and delivery room , operational room for cesarean section and maternity word in maternity department at Al Emamain Al Kadhamain Medical City in Baghdad city. Data collection was initiated on 2nd January to end of March /2014. Purposive sample consisted of (150) mother and her neonate, The study sample divided into three groups:(50) under general anesthesia , (50) under spinal anesthesia ,(50) normal vaginal delivery and considered control group. An assessment tool was constructed for the purpose of the study, it was comprised of socio-demographic data, mother reproductive history, information about neonate age, weight, Apgar score, anesthesia effect on neonate after delivery and assessment of breast feeding immediately after birth and anesthesia effect on bonding. Results : The main results of the study shows that the highest percentage of neonate age at ( 1 & 2) hours after delivery, , Neonate weight 3000 gm., general group had illustrated low Apgar scoring scale comparing with others groups. Mother choice of anesthesia, odds ratio indicator showed that choice of general anesthesia twice times of choice spinal anesthesia .Regarding mother practice of breast feeding, mother in general anesthesia started breastfeeding after two attempts or more ,while in N.V.D. started from first attempt, for the reasons of inability to achieve breast feeding, all items in general reported failure assessment, also in and spinal groups all items reported failure except in Feeling severe pain after the operation", " The mother in recovery room immediately after the operation", and "Lack of milk flow as a result of anesthesia" which reported pass assessment. Finally, N.V.D. has reported pass assessment in light of preceding part. Reasons related to child", general and spinal groups are reported failure assessments in light of "A child needs to transfer to the intensive care unit, and Baby feeling tired and not being able to breast feeding" items, and the leftover items are reported pass assessments. While, N.V.D. has reported pass assessment in all items. Effectiveness of anesthesia on breastfeeding shows failure assessment in general and spinal group in mother practice , reasons related to mother and newborn, while pass in N.V.D. group. Regarding mother-neonate bonding ,results of effectiveness shows failure assessment in general and pass in spinal and N.V.D. group.. Recommendations: Early mother-neonate bonding appears to offer some clinical benefit, particularly for breastfeeding. Supportive policies are needed in hospitals and birthing centers to help implement this intervention, which might be regarded as "natural".

To date, the main cause of maternal mortality rate is bleeding. Placenta previa is one of the causes of bleeding in pregnant women. This study was aimed to obtain the relationship between the occurence of placenta previa and pregnancy history among patients at RS Bhayangkara Manado, RSU GMIM Pancaran Kasih Manado, Prof. Dr. R. D. Kandou Hospital Manado. This was a descriptive and retrospective study with a cross sectional design. Subjects were pregnant women or women who laboured from January 2017 to December 2018 that had placenta previa. There were 72 cases as subjects, obtained by using non random sampling. The results showed that placenta previa were most common among subjects aged ≥35 tahun as many as 30 subjects (41.7%), multiparity as many as 39 subjects (54.2%), no history of sectio caesarea as many 39 subjects (54.2%), and no history of curetage as many as 66 subjects (91.7%). In conclusion, there were relationships between the occurence of placenta previa and age ≥35 years as well as multiparity, albeit, there were no relationships between the occurence of placenta previa and sectio caesaria as well as curetage history.Keywords: placenta previa, age, parity, histories of sectio caesarea and curetage Abstrak: Penyebab angka kematian ibu (AKI) yang utama ialah perdarahan. Plasenta previa merupakan salah satu penyebab perdarahan yang tersering terjadi pada ibu hamil. Penelitian ini bertujuan untuk mengetahui hubungan kejadian plasenta previa dengan riwayat kehamilan sebelumnya di RS Bhayangkara Manado, RSU GMIM Pancaran Kasih Manado dan RSUP Prof. Dr. R. D. Kandou Manado. Jenis penelitian ialah deskriptif retrospektif dengan desain potong lintang. Subyek penelitian ialah ibu hamil atau bersalin yang mengalami plasenta previa pada periode Januari 2017-Desember 2018 yang berjumlah 72 kasus, diperoleh dengan metode non random sampling. Hasil penelitian menunjukkan kejadian plasenta previa terbanyak pada usia ibu >35 tahun yaitu 30 orang (41,7%), paritas multipara yaitu 39 orang (54,2%), tidak ada riwayat seksio sesarea yaitu 39 orang (54,2%), dan tidak ada riwayat kuretase yaitu 66 orang (91,7%). Simpulan penelitian ini ialah terdapat hubungan kejadian plasenta previa dengan usia ibu dan multiparitas namun tidak terdapat hubungan dengan riwayat seksio sesarea dan riwayat kuretase.Kata kunci: plasenta previa, umur ibu, paritas, riwayat seksio sesaria dan riwayat kuretase

Introduction: Placenta praevia complicates approximately 0.3-0.8% of pregnancies in multiparous parturient and there is an increased risk in those with previous Caesarean section. The choice of anaesthetic technique usually poses dilemma to the attending anaesthetist. The aim of this study was to review the choice of anaesthetic technique for caesarean section complicated by placenta praevia and determine foeto-maternal outcome.Patients and Methods: This was a retrospective survey covering a period of four years (January 2013 - December 2016). After Ethics Committee approval, information was obtained from the anaesthetic and theatre records and the patients’ case notes. The information obtained were: patients’ characteristics, obstetric history, grade of placenta praevia, indications for surgery, choice of anaesthetic technique, PCV/haemoglobin level, haemodynamic changes, blood transfusion, maternal and foetal outcome. The data were analysed using IBM SPSS version 20.Results: A total of 4,388 Caesarean sections were carried out during the 4- year period of review. One hundred and forty-five cases (3.3%) with different degrees of placenta praevia progressing to Caesarean section were identified. The mean age (years) was 32.6+5.3 with a mean gestational age(weeks) of 36.7±1.11. Thirty-nine (26.9%) patients were offered general anaesthesia while 106 (73.1%) were done under spinal anaesthesia. The mean blood loss in the GA and spinal group was 1,080 + 970.53 and 753.77 + 568.19 ml respectively (P = 0.0135). 87.6% neonates were delivered alive. Seventy-nine (54.5%) were elective procedure with adequate preparation. There were two (1.4%) cases of maternal mortality and perinatal mortality of 12.4%.Conclusion: The anaesthetic technique for patients with placenta praevia should be determined by the patient’s haemodynamic status and the grade of the placenta praevia.
