Choledochal cysts in adults and the importance of differential diagnosis

Abstract: Contemporary approaches to treating choledochal cysts focus on total removal of the cyst. In a country where hydatic diseases are endemic, hepatobiliary cystic lesions are often misdiagnosed clinically and radiologically as hydatid cysts. Considering that percutaneous and laparoscopic approaches are being increasingly adopted for treatment of hydatid cysts, the problematic area must be subjected to thorough examination for cystic lesions. The possibility of choledochal cysts must always be kept in mind; otherw… Show more

“…Floating undulating membranes on ultrasound,10 or multiple septa within a calcified cyst on CT prove particularly helpful in the diagnosis of gallbladder hydatid cysts. However, imaging findings of hydatid cysts are not always specific, and differential diagnosis should be made from other cysts, such as choledochal cysts, pancreatic pseudocysts, abscesses, and cystic neoplasms 13,14. Serological tests could help to delineate the cyst's nature.…”

The gallbladder cyst size at diagnosis was always small (maximal diameter, Ͻ5 cm).…”

“…71 For diagnosing choledochal cysts, US is the modality of choice. The classic presentations of adult patients are jaundice, pain, and palpable mass, but some patients also present with cholangitis symptoms including fever or vague dyspeptic symptoms.…”

“…Biliary cysts can be mistaken for hydatid cyst in endemic regions. This was the issue of our previous study (11). We focused on this issue after we found a solitary intrahepatic cyst extending outside the surface of the liver in a patient who was taken into operation with hydatid liver cyst diagnosis.…”
