Chronic pain and psychedelics: a review and proposed mechanism of action

Abstract: The development of chronic pain is a complex mechanism that is still not fully understood. Multiple somatic and visceral afferent pain signals, when experienced over time, cause a strengthening of certain neural circuitry through peripheral and central sensitization, resulting in the physical and emotional perceptual chronic pain experience. The mind-altering qualities of psychedelics have been attributed, through serotonin 2A (5-HT2A) receptor agonism, to ‘reset’ areas of functional connectivity (FC) in the b… Show more

“…One possible mechanism of action of this analgesic property is via interaction with nociceptive and antinociceptive pathways [ 48 ]. In some cases, psilocybin was comparable to or more efficacious than traditional medications such as opioid analgesics [ 6 , 50 ].…”

“…Parallel with the pandemic, several studies have been published regarding the potential therapeutic effects of psychedelic drugs, such as ayahuasca, psilocybin, and lysergic acid diethylamide (LSD) (24)(25)(26)(27), especially for the treatment of mental health issues (25,(28)(29)(30). Other studies have shown their potential for the treatment of pain (31,32) and neurodegenerative disorders (33,34). These benefits have also been observed in real-world situations through large population surveys, where psychedelic drug users have been shown to have a lower rate of mental health problems (35,36), reduced psychological distress and suicidality (37), and better outcomes in terms of internationally-validated health indicators (38).…”

“…It also poses an incredible burden on not only individuals but also the health care system. 4 Unfortunately, management of NP remains a major clinical challenge, given that the pathogenesis of NP is not yet completely elucidated. 5 Moreover, regardless of the inciting cause of NP it always shares the clinical characteristics previously described.…”
