Ciliated hepatic foregut cyst with extensive squamous metaplasia: report of a case

Abstract: Ciliated hepatic foregut cysts (CHFC) are rare cystic lesions of the liver composed of a ciliated pseudostratified columnar epithelium with mucous cells, connective tissue, and smooth muscles bundles. We report the first case of CHFC with extensive squamous metaplasia without dysplasia or carcinoma. A unilocular, avascular, hypoechoic 60-mm liver lesion located in segment IV was detected by ultrasonography in a 31-year-old woman. The cyst was surgically removed and was lined mainly by a regular squamous epithe… Show more

“…One case had concomitant squamous cell carcinoma as well as gastric antral metaplasia, and one case had only gastric metaplasia [ 16 , 33 ]. Tumor marker serologies were performed in 4 of the metaplastic/malignant cases (3 squamous cell carcinoma, 1 squamous metaplasia), and all were within normal limits [ 10 , 13 , 15 , 17 ]. Our series reports the third case of gastric metaplasia arising within a CHFC, and it is the second known case of gastric metaplasia in CHFC not associated with malignancy.…”

“…However, there have been cases reported with presentations of portal hypertension [18], jaundice [19], elevated liver enzymes [16,20], and right upper quadrant abdominal pain [9,12,17], which is typically moderate in severity and vague or gradual in onset. The sudden, severe pain observed in the current case is therefore unusual, and is intuitively, if not causally, related to the occurrence of hemorrhage, which is itself an uncommon finding, observed in only 6.2% of reported cases.…”

“…The most common histology was squamous cell carcinoma in 4 (80%) of the 5 cases of malignancy [7][8][9][10]; the other one was a cystadenocarcinoma arising within a CHFC [11]. Extensive squamous metaplasia was reported in one case [12]. The size of cysts that contained a malignancy (median 10 cm, range 3.2 -13 cm) was significantly greater that those that did not (median 3 cm, range <1 -11 cm, P < 0.01 by two-tailed Mann-Whitney test) (Figure 3).…”

“…They are generally asymptomatic benign lesions and they generally do not communicate with the gallbladder although there is only one case communicating with the gallbladder [7]. Squamous metaplasia and even squamous cell carcinoma may arise in a hepatic foregut cyst [8, 9]. Laparoscopic excision is preferred for the treatment, as a minimally invasive procedure [10].…”
