Glomerulonefrite membranoproliferativa: classificação histopatológica atual, perfil clínico e desfechos renaisIntrodução: Glomerulonefrite membranoproliferativa (GNMP) é uma doença glomerular rara com prognóstico variável. Recentemente, foi proposta uma nova classificação baseada na presença ou ausência de imunoglobulinas e depósitos de complemento na microscopia de imunofluorescência (IF) da biópsia renal. Os objetivos do estudo foram determinar e comparar as características clínicas, laboratoriais e histopatológicas daqueles com GNMP primária ou secundária, reclassificar as primárias com base em achados da IF e avaliar os desfechos renais. Métodos: Este foi um estudo de coorte observacional retrospectivo realizado em centro único (UNIFESP), com base nos dados coletados de prontuários de pacientes acompanhados de 1996 a 2019. Resultados: Dos 53 casos de GNMP, 36 (67,9%) foram classificados como GNMP primária e 17 (32,1%) como GNMP secundária. A maioria dos pacientes era hipertensa (84,9%) e apresentava edema (88,7%) e anemia (84,9%); 33 (91,7%) pacientes classificados como GNMP primária foram reclassificados como mediados por imunocomplexo e 3 (8,3%) como mediados por complemento. O grupo de GNMP secundária apresentou mais frequentemente hematúria (p <0,001) e maior prevalência de depósitos de IgG (p = 0,02) e C1q (p = 0,003). Com relação ao desfecho, 39% dos pacientes alcançaram remissão parcial ou completa. Albumina sérica inicial mais baixa e proteinúria de 24 horas inicial mais elevada foram fatores associados a pior prognóstico renal. Conclusões: De acordo com a nova classificação histológica, a grande maioria dos casos de GNMP foram classificados como sendo mediados por imunocomplexos. Houve poucas diferenças entre GNMP primária e secundária em relação às suas características clínicas e laboratoriais.

Diagnosis and management of complicated urogenital schistosomiasis: a systematic review of the literature

Background Currently, there are no standardized guidelines for the diagnosis or management of the complications of urogenital schistosomiasis (UGS). This systematic review of the literature aims to investigate the state of the art in reference to diagnostic approaches and the clinical management of this condition. Methods A systematic review of literature published between January 1990 and January 2021 was conducted in the MEDLINE database, scoping for articles regarding diagnostic means or therapeutic options for the complications of UGS, namely obstructive uropathy, bladder cancer, abortion, ectopic pregnancy, infertility, kidney failure, urolithiasis and the need for invasive procedures. Relevant data were then extracted from the articles deemed eligible according to the inclusion criteria. Main results In total, 3052 articles were identified by the research query, of which 167 articles fulfilling inclusion criteria after title/abstract screening and full-text evaluation were included, 35% on both diagnostic and therapeutic aspects, and 51% on diagnosis and 14% on therapy. Ultrasound was the most frequently tool employed for the diagnosis of UGS complications showing a good performance. Concerning the management of hydronephrosis, the majority of available evidences came from community-based studies where universal treatment with praziquantel was used leading to decrease of prevalence of obstructive uropathy. Concerning studies on surgical procedures, laser endoureterotomy followed by stenting was mostly employed in adult patients leading to a crude cure rate of 60% (43 of 71 patients). In the case of severe hydronephrosis, surgery consisting of ureteral re-implantation showed excellent results with a crude cure rate of 98% (157 cured patients of 160 treated). Concerning bladder cancer, data on 93 patients with a clear diagnosis of UGS-related bladder were available reporting a variable and sometime combined approach based on disease stage. Available data on diagnosis and management of abortion, ectopic pregnancy, infertility, kidney failure, urolithiasis and the need for invasive procedures due to UGS are also presented. Conclusions The review produced a complete picture of the diagnostic and therapeutic options currently available for complicated UGS. These results can be useful both for guiding clinicians towards correct management and for tracing the direction of future research.

A glomerulopatia C3 é uma doença renal considerada rara. A patologia é ocasionada pela ativação excessiva da via alternativa do sistema complemento da circulação. O diagnóstico da GC3 é baseado na clínica do paciente, exames complementares e biopsia renal por imunofluorescência (IF) e microscopia eletrônica que devem apresentar deposição glomerular de C3. A intervenção terapêutica é realizada por direcionamento do sistema complemento em GC3 e com imunossupressores em GN mediada por Ig/IC. O tratamento fundamenta-se também em medidas de renoproteção. Esta pesquisa foi do tipo teórica, exploratória, descritiva com objetivo de levantar informações sobre o tema: Glomerulopatia por C3, perspectivas sobre o tratamento. Foram incluídos artigos publicados no período de dez anos -2012 a 2021, presentes na base de dados Lilacs (Índice da Literatura Científica e Técnica da América Latina e Caribe), Medline (Literatura Internacinal) e Scielo (Scientific Eletronic Library Online), que se encontram na Biblioteca Virtual da Saúde (BVS-Bireme), AcademicSearch e Google Acadêmico. O tratamento atualmente mais eficiente é uma combinação de drogas imunossupressoras ou agentes anticomplemento com intuito de induzir remissão parcial ou completa da GC3. Terapias mais recentes, como anticorpos monoclonais e proteínas recombinantes podem ser estratégias promissoras em um futuro próximo. A abordagem terapêutica da GC3 integra intervenções específicas a tratamentos não específicos, com objetivo
