Clinical Aspects of Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis With Severe Ocular Complications in Taiwan

Abstract: Purpose: Over the last decade, there has been tremendous progress in the treatment of Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN). To understand whether this has resulted in better ophthalmic outcomes, we aimed to study the incidence of severe ocular complications (SOCs) in the acute and chronic stage among SJS/TEN patients, major causative medications, and therapeutic effect of medical and surgical treatment. Methods: Using electronic medical records review of patients of Chang Gung Mem… Show more

“…Several studies attempted to circumvent these limitations by extracting data from electronic health records (EHRs) and large repositories (9)(10)(11)(12)(13)(14). For example, Micheletti et al (11) performed a retrospective cohort study, notably collecting data across 18 United States medical centers and identified 377 SJS/TEN cases from EHRs.…”

“…The close relationship between the severity of acute ocular discomfort and chronic ocular complications was emphasized. Even though the patients initially only experienced mild ocular symptoms in the acute stage, chronic ocular sequelae may occur later 35 . Thus, prompt ophthalmological consultation and regular long‐term ophthalmological follow‐up are necessary for those with ocular involvement to relieve or treat the associated symptoms.…”

Various forms of nail complications were also observed in the acute stage and long-term follow-up, including nail dystrophy and onychomadesis as acute complications, and longitudinal nail riding and V-shaped notching as long-term sequelae, which occurred significantly more often in pediatric patients with TEN.…”

“…Dermatologic toxicities seem to be the most frequently reported adverse events. SJS is a rare and severe dermatologic toxicity with high mortality[ 4 ]. The first SJS case induced by pembrolizumab in NSCLC was reported in a Japanese case[ 14 ].…”

“…Stevens-Johnson syndrome (SJS) is a severe type of pleomorphic erythema and a rare adverse mucocutaneous reaction with a mortality rate of up to 35%[ 3 ]. While SJS is characterized by maculopapular rash – pruritus – and is often related to adverse drug reactions[ 4 , 5 ], the mechanism of SJS has not been determined. It has been reported that at least 200 kinds of drug reactions are related to SJS.…”
