Clinical Characteristics, Treatment Effectiveness, and Predictors of Response to Pharmacotherapeutic Interventions Among Patients with Herpetic-Related Neuralgia: A Retrospective Analysis

Abstract: Background:The treatment for herpetic-related neuralgia focuses on symptom control by use of antiviral drugs, anticonvulsants, and tricyclic antidepressants. We aimed to explore the clinical characteristics associated with medication responsiveness, and to build a classifier for Jiehua Zhou and Wuping Sun contributed equally to this work.

“…Notably, ML models also provide a novel identification method for patients with herpetic neuralgia who are at risk of inadequate pain management. Studies have explored the clinical characteristics of herpetic neuralgia patients with medication responsiveness and built an optimal ML model to discriminate medication-resistant pain (MRP) from medication-sensitive pain (MSP) ( Zhou et al, 2021 ). However, this study has some limitations.…”

“…Medication-resistant PHN is a further challenge. A Chinese study evaluated potential risk factors and identified subacute HZ (vs. acute HZ; odds ratio (OR): 9.0), severe lesions (vs. mild lesions; OR: 3.8), and depressed mood (OR: 1.1) as risk factors [46].…”

“…The present analgesic treatments for HN are still limited due to the poor understanding of its pathogenic mechanism. It has been reported that age, immune deficiencies, malignancies, organ transplantations, autoimmune diseases, emotional stress, and immunosuppressive therapies are the major risks for HN 9,10 . Clinical treatment for HN only focuses on symptom control, including the use of topical lidocaine or capsaicin and oral gabapentin, pregabalin, or tricyclic antidepressants, 11,12 as well as the improvement of immunity and antiviral treatments 13,14 …”
