Background: Sphingomonas paucimobilis is a non-fermentative bacillus and found widely in nature. It acquires great interest in biofouling by production of biofilm. However, S. paucimobilis as a biofilm producer, is not studied in medical aspect. In this study we aimed to assess the biofilm production as a virulence factor in clinical isolates of S. paucimobilis together with patient demographics, clinical aspects, risk factors, and outcomes. Methods: During 5 year-period, total numbers of 43 S. paucimobilis isolates identified in a clinical microbiology laboratory of an university hospital in Turkey. Thirty-three of the isolates, which were isolated from patients with clinically determined infection, were enrolled in this study. Patients' data were collected retrospectively. VITEK II automated system (bioMérieux, Marcy L'Eoile, France) was used for identification and antimicrobial susceptibility testing. Christiansen tube method was used to determine biofilm formation. Results: All the clinical isolates of S. paucimobilis produced biofilms. Primary bacteraemia (n = 16) was the most common clinical manifestation. Twenty-four of the patients were followed in the intensive care unit. Twenty-two patients had indwelling catheters. Malignancy (n = 11) and diabetes mellitus (n = 10) were the most common concomitant diseases. Tigecycline, carbapenems and aminoglycosides were the most susceptible antimicrobial agents. Degree of biofilm formation was correlated only with blood samples (P = 0.003) in the sample types group and a stay in the intensive care unit (P = 0.002) in the risk factors group. Conclusions: Sphingomonas paucimobilis can cause serious infections, especially in immunocompromised patients with determined risk factors such as indwelling catheters and diabetes mellitus, due to the effects of multiple virulence factors, together with biofilm formation.

ÖZETSphingomonas paucimobilis genellikle altta yatan bir komorbit hastalık ile beraber patojenite gösteren fırsatçı bir enfeksiyon ajanıdır. Sphingomonas paucimobilis bakteriyemi, ventilatör ilişkili pnömoni, intravasküler kateter ilişkili enfeksiyonlar, menenjit, peritonit, osteomyelit, septik artrit, postoperatif endoftalmit, plevral ampiyem, splenik apse, üriner trakt enfeksiyonları ve bilier trakt enfeksiyonları da dahil olmak üzere çok çeşitli enfeksiyonlarla ilişkilendirilmiştir. Bu vaka takdiminde osteosarkoma nedeni ile endoprostetik tümör rekonstrüksiyonu yapılan bir olguda, Sphingomonas paucimobilis'in neden olduğu prostetik eklem enfeksiyonu bildirilmiştir. Bu olgu S. paucimobilis'in neden olduğu, tümör rezeksiyon protezi eklem enfeksiyonu raporudur. Anahtar Kelimeler: Sphingomonas paucimobilis, Endoprostetik enfeksiyon, Osteosarkoma ABSTRACT Sphingomonas paucimobilis is an opportunistic infectious agent that generally shows its pathogenicity by accompanying an underlying comorbid disease. It is recognized that Sphingomonas paucimobilisis associated with many infections, including: bacteremia, ventilator related pneumonia, intravascular related catheter infections, meningitis, peritonitis, osteomyelitis, septic arthritis, postoperative endophthalmitis, pleural empyema, splenic abscess, urinary tract infections and biliary tract infections. The present report described a prosthetic joint infection, caused by Sphingomonas paucimobilis, in a patient who had endoprosthetic tumor reconstruction due to osteosarcoma. It chronicles the prosthetic joint infection with endoprosthetic tumor reconstruction in the literature caused by S. paucimobilis.

Incidence and molecular characterization of multidrug resistance in Gram‐negative bacteria of clinical importance from pharmaceutical wastewaters in South‐western Nigeria

Background The occurrence of antimicrobial resistant bacteria in the environment presents a major threat to public health because it reduces the effectiveness of antimicrobial treatment. Aims The study was set out to molecularly characterize Gram‐negative bacteria with multidrug resistance and resistance determinants from pharmaceutical wastewaters in Nigeria. Materials and Methods Susceptibility of the bacterial isolates to 25 antibiotics belonging to 10 categories was tested using the disc diffusion method and Vitek 2. Screening for AmpC, Extended Spectrum Beta‐lactamase and carbapenemase production was done by Polymerase Chain Reaction and sequencing. Results Ninety‐seven Gram‐negative bacteria, comprising 27 Enterobacteria and 70 nonfermenter bacterial isolates were detected. Antibiotic resistance observed was highest (70.1%) for sulfamethoxazole / trimethoprim and multidrug resistance was revealed in 17 bacterial strains (Klebsiella pneumoniae [7], Enterobacter gergoviae [3], Sphingomonas paucimobilis [1], Empedobacter brevis [1], Chryseobacterium indologenes [1], Pseudomonas aeruginosa [1], Burkholderia cenocepacia [1], Burkholderia cepacia and Stenotrophomonas maltophilia [1]). Extended Spectrum Beta‐lactamase (CTX‐M‐15, SHV‐12, SHV‐2) was positive for 6 K. pneumoniae strains; there were neither AmpC detected nor the production of carbapenamase in all isolates tested. Discussion The study confirmed the presence of multidrug resistant Gram‐negative bacteria with resistance determinants in wastewaters from pharmaceutical industries in Nigeria. Compounds of the wastewater may directly select or co‐select these multidrug resistance strains. Conclusion The output of drug resistant bacteria into the environment is a potential risk to public health and may facilitate the spread of resistant genes.
