Clinical Islet Transplantation: A Review

Abstract: For decades, the inability of insulin therapy to physiologically control glycemia in type I diabetic patients has motivated the search for insulin-delivering grafts. Islet autotransplantation is such a therapeutic approach to prevent diabetes mellitus following a major pancreatectomy, whereas allotransplantation is generally prescribed for type I diabetic patients with a functional solid organ graft, or for patients awaiting one. As of today, over 150 patients have been autotransplanted world-wide, following t… Show more

“…While the HBPA-GF cure percentage did not surpass that of the IH group, the results illustrate the potential utility of HBPA scaffolds in developing alternatives to intrahepatic implantation. Alternative islet engraftment sites can potentially address concerns associated with portal venous puncture and post-engraftment liver damage(2527), and may offer additional advantages, such as the accommodation of larger volumes of tissue(4) and insulin delivery via the portal venous circulation(28). …”

“…The purified islet suspension is slowly infused with continuous monitoring of the intraportal hydrostatic pressure. A significant elevation of the intraportal pressure above the few millimeters of mercury routinely observed commands immediate cessation of the procedure [10].…”
