Growth Factor Delivery From Self-Assembling Nanofibers to Facilitate Islet Transplantation

Recent advances in nanotechnology and molecular self-assembly may provide novel solutions to current cell transplantation deficiencies. Heparin-binding peptide amphiphiles (HBPAs) self-assemble from aqueous media into nanofibers that bind growth factors through interactions with the bioactive polymer heparin. In this report, we demonstrate that delivery of vascular endothelial growth factor (VEGF) and fibroblast growth factor-2 (FGF-2) from HBPA scaffolds significantly increases blood vessel density in the mouse omentum over control scaffolds without growth factors (P<0.0005) and significantly enhances islet engraftment. Diabetic recipients transplanted with 250 isologous islets and HBPA scaffolds containing VEGF/FGF-2 achieved normoglycemia at a higher rate (78%) than control animals receiving identical scaffolds without growth factors (30%; P<0.05) or growth factors alone (20%). These data indicate that the enhanced engraftment can be attributed to specific growth factor effects that were made possible by the delivery mechanism of HBPA nanostructures.

Identification of obstacles to the success of islet transplantation, such as primary nonfunction, immunosuppression-related metabolic workload, or recurrence of autoimmunity, will allow the development of new strategies tailored to overcome them. In particular, novel immunosuppressive protocols, with or without the aim of inducing tolerance, and immunoisolation devices are reaching the stage of clinical applicability. Finally, several strategies, such as utilization of porcine xenogeneic islets or genetically engineered beta-cell lines, or in vitro expansion of cultured beta-cells could theoretically expand indefinitely the pool of islet tissue for transplantation.

Accelerated developments and improved understanding of the issues that face clinical islet cell transplantation during the last 20 years have led this simple concept to a successful treatment for diabetes. Islet cell transplantation involves the extraction of islets of Langerhans from organ donors through complex digestion and purification processes. After implantation in patients with type-1 diabetes, the treatment can provide near perfect, moment-to-moment control of blood glucose, far more effectively than injected insulin. The procedure offers the benefits of whole pancreas transplantation, but with less risk. Since the introduction of "Edmonton Protocol", significant advances in islet isolation techniques and purification technology, novel immunosuppressants and tolerance strategies have renewed interest in clinical islet transplantation for the treatment of diabetes mellitus. The "Edmonton Protocol" has been successfully replicated by other centers in an international multicenter trial. A number of key refinements in pancreas transportation, islet preparation and newer immunological conditioning and induction therapies have led to continued advancement through extensive collaboration between key centers. This article provides an overview of the history of islet transplantation followed by discussion on current controversies of donor selection, pancreas procurement, hypothermic preservation solutions, current islet isolation technique, islet transplantation procedure, post transplantation immunology, and developments for future.
