Clinical Presentation, Staging and Long-Term Evolution of Parathyroid Cancer

Abstract: Understaging and undertreatment are shown to contribute to high recurrence rates and death toll. To improve outcome, en bloc resection including central lymph node dissection should be the minimal surgical approach in any patient with suspected parathyroid cancer.

“…Guidelines indicate that parathyroid lesions >3 cm in patients with serum Ca concentrations >3 mmol/l (>12 mg/dl) should be suspected of being PC [17]. Serum Ca concentrations in patients with PC range from 14.6 to 15.9 mg/dL (3.7 to 4.0 mmol/L), with up to 75% having albumin-corrected Ca concentrations >14 mg/dL (>3.5 mmol/L) [18]. The serum Ca concentration of the patient described here was 12.4 mg/dL (>3 mmol/L).…”

“…Distant recurrence is equally common, and the majority of distant metastases occur in the lungs, mediastinum, and bone. 1 Prophylactic central neck dissection may be beneficial from an aggressive surgical approach when feasible in patients with recurrent or metastatic parathyroid carcinoma. 63 The purpose of the surgical operation is to remove all gross disease in the neck, mediastinum, and distant sites if possible.…”

“…1 Less than 10% of cases of parathyroid carcinoma are non-functional, 2 presenting with normal levels of parathyroid hormone. De Quervain, 3 described the first known case in 1909, and to date, few cases have been reported in the literature.…”
